JOB TITLE: COMMUNITY HEALTH NURSE SUPERVISOR

JOB CODE: 30830

DEFINITION: This is supervisory professional nursing work in a community health district or state health center.

An employee in this job is responsible for supervising community health nursing staff in organizing, promoting, and participating in the operation of a public health program for the purpose of preventing, controlling, and eliminating public health problems or diseases. Work also involves supervising the direct delivery of clinical nursing services within the community, either in a state health center or through home visits to individuals and families. Work includes the identification of staff development needs, the evaluation and initiation of developmental training recommendations for nursing personnel, and the appraisal of the effects developmental training has on overall performance of the nursing staff. Work is received in the form of oral or written instructions and employees exercise independent judgment in performing supervisory duties. Work is reviewed by a district community health nurse administrator through reports, conferences, meetings, and observation for effectiveness and compliance with standards. • Directs the community health nursing program in an assigned area of the district.

• Directs the provision of clinical nursing services in a state health center or through home visits.

• Supervises the organization of or participation in various activities with community partners to promote public health program objectives and further community involvement and ownership.

• Supervises the organization of meetings to facilitate interaction and encourage networking between community partners.

• Directs staff that serve as a committee or board member for community partners.

• Conducts needs assessments.

• Collects, organizes, and analyzes public health statistics and survey results.

• Presents results of needs assessments, recent public health developments and trends, and identified objectives.

• Directs the development of program specific activities based on needs assessments and consultation with staff, community partners, healthcare providers, and the public.

• Provides technical advice on laws, regulations, and procedures.

• Provides guidance in the completion of grant applications and grant compliance.

• Responds to inquiries from healthcare providers, internal and external staff, community partners, and the public.

• Directs the distribution of pamphlets, brochures, equipment, and supplies to healthcare providers, community partners and organizations, and the public.

• Plans and presents in-service training and educational programs to district and state health center staff, community partners, healthcare personnel, and the public.

• Plans and directs workshops, institutes, forums, and discussion groups to promote public health.

• Supervises the conduct of epidemiological investigations and follow-ups.

• Performs assessments to identify abuse, neglect, and developmental problems.

• Supervises the provision of counseling, education, and demonstrations on physical care methods to patients and families.

• Supervises the development of nursing care plans.

• Supervises the monitoring of contracts on behalf of and in coordination with central office personnel.

• Reviews provider reports.

• Reviews forms for accuracy and completeness.

• Provides follow up through plans of correction and technical assistance.

• Supervises the response to district or statewide emergency situations.

• Performs related work as required.

ENTRY LEVEL KNOWLEDGES, SKILLS, AND ABILITIES:

• Knowledge of the theories and practices of professional nursing.

• Knowledge of the theories and practices of professional community health nursing.

• Knowledge of the principles and practices involved in organizing and directing the work of others.

• Knowledge of the purpose and functions of other health care and social organizations in the community.

• Knowledge of current developments in the field of community health nursing.

• Knowledge of research methods including statistical evaluation.

• Knowledge of the principles and techniques utilized in the employee interview and selection process.

MINIMUM EXPERIENCE AND TRAINING: (NOTE: Based on the Entry Level Knowledges, Skills, and Abilities)

• Three years as a Community Health Nurse (commonwealth title);

or

• Four years of community health nursing experience;

or

• An equivalent combination of experience and training.

SPECIAL REQUIREMENTS:

• All positions require active authorization to practice as a Registered Nurse in Pennsylvania.

• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.
