SOCIAL WORK DISCIPLINE COORDINATOR, DMVA

DEFINITION: This is professional social work in planning, developing, and coordinating all social work programs for the Department of Military and Veterans Affairs (DMVA).

The employee in this job oversees the provision of counseling services, social services, and case management services in all veterans’ homes operated by DMVA. Work includes planning, developing, and implementing social work programs, policies and procedures, integrating social work services with the needs and requirements of other veterans’ homes programs, and evaluating the provision of social work services for overall effectiveness and compliance with applicable rules, regulations, policies, and laws. Work also involves advising veterans’ home-level and administrative staff on social work policies and issues. Supervision may be exercised over technical or professional staff. Work is performed under the general direction of the Chief, Division of Health Care Services and is reviewed through conferences, reports, and results achieved.

EXAMPLES OF WORK: Plans, develops, and establishes social work program policies, standards, and priorities for facilities providing services to residents of extended care facilities.

Develops, implements, and monitors social work programs such as admissions, assessments, patient screening, case management, and treatment planning.

Provides guidance and technical assistance to veterans’ home-level social work staff to resolve unique or specialized situations.

Monitors and evaluates the quality of work performed by social work staff to ensure maintenance of appropriate levels of care.

Interprets social work program policies for veterans’ homes staff and community agency representatives.

Ensures veterans’ homes comply with federal and state regulatory requirements.

Reviews aggregate resident admission, readmission, or discharge data and documentation to identify and address social services trends with appropriate parties.

Arranges for diagnostic reviews in accordance with appropriate legislation or regulatory requirements.

Ensures social service discharge case closing and transfer plans are developed to summarize residents’ disposition and need for continued aftercare or follow-up services upon discharge.

Participates in multidisciplinary meetings to develop and implement integrated care policies, procedures, and plans.

Coordinates appropriate referrals, onsite visits, and service inspections with veterans’ home medical staff to effectuate maximum care and treatment.

Performs the full range of supervisory duties.

The employee in this job may participate in the performance of subordinates’ work consistent with operational or organizational requirements.

REQUIRED KNOWLEDGES, SKILLS, AND ABILITIES: Knowledge of professional social work principles, practices, and methods.

Knowledge of professional casework principles, practices, and methods.

Knowledge of social, economic, health, habilitative, and rehabilitative problems, treatment methods, and resources.

Knowledge of individual and group behavior.

Knowledge of counseling techniques and interventions used to recognize or improve social, economic, emotional, behavioral, and health situations of individuals and families.

Knowledge of federal and state rules, regulations, standards, policies, and laws that relate to social work services and programs.

Knowledge of the external organizations and agencies which provide client social services.

Knowledge of the requirements of the Health Insurance Portability and Accountability Act (HIPAA) as applied to social work practice.

Ability to analyze and interpret resident case records to ensure compliance with social work program policies and procedures.

Ability to maintain effective working relationships.

Ability to communicate effectively in writing.

Ability to communicate effectively orally.

MINIMUM EXPERIENCE AND TRAINING: One year of experience as a Social Worker 2; and a master’s degree in social work or social welfare;

Three years of professional social work experience, including one year in a consultative, administrative, advisory, or lead work capacity; and a master’s degree in social work or social welfare.

Necessary Special Requirement: Possession of a valid license as a Licensed Social Worker or Licensed Clinical Social Worker issued by the Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors.
