JOB TITLE: CONSUMER TRANSITION SUPPORT DIRECTOR, DHS

DEFINITION: This is professional and administrative work directing and developing policy for a program to rebalance and improve long-term care support systems to enable people of all ages with disabilities or long-term illnesses to live meaningful lives and participate in their communities.

The employee in this job is responsible for planning, directing, controlling, and implementing the Money Follows the Person Demonstration Program. Work involves advising executive management and collaborating with stakeholders to make widespread changes to the long-term care support systems in Pennsylvania; developing and revising policies to improve community-based services and community integration; and eliminating barriers in programs, procedures, and policies so that financing, services, and supports move with a person to an appropriate and preferred setting. The employee works in close collaboration with executive management to coordinate project activities, gather information, and resolve issues of program and program policy conflict. Work is performed under the general direction of an administrative supervisor through conferences, reports, and an evaluation of overall effectiveness.

• Establishes policies, priorities, and direction for the Money Follows the Person Demonstration Project.

• Advises executive management staff and works in collaboration with agency Deputy Secretaries, the Chief Financial Officer, and cabinet officials in other agencies in making widespread changes to long-term care support systems and resolving policy conflicts.

• Plans, directs, and coordinates all program activities to ensure that goals and objectives are accomplished within the prescribed time frame and funding parameters.

• Manages program operations and monitors progress towards objectives, establishes internal and external reporting systems, and develops corrective action plans.

• Serves as the agency’s primary point of contact with the Stakeholders Leadership Group, the Governor’s Council for Long-Term Living advisory groups, the federal Department of Health and Human Services, and the Centers for Medicare & Medicaid Services.

• Prepares status reports as required.

• The employee in this job may participate in the performance of subordinates’ work consistent with operational or organizational requirements.

• Performs the full range of supervisory duties.

• Performs related work as required.

ENTRY LEVEL KNOWLEDGES, SKILLS, AND ABILITIES:

• Knowledge of the principles and practices of public administration.

• Knowledge of the regulations governing the administration of human services programs.

• Knowledge of the functions and resources of human services organizations and programs.

• Knowledge of the principles and methods of policy and program planning, development, and evaluation.

• Ability to analyze and evaluate data.

• Ability to read and interpret written documentation.

• Ability to establish and maintain effective working relationships.

• Ability to communicate effectively orally.

• Ability to communicate effectively in writing.

MINIMUM EXPERIENCE AND TRAINING: (NOTE: Based on the Entry Level Knowledges, Skills and Abilities)

• Four years of professional experience in a human services agency, health care insurance program, or health care services, which included two years of experience involving planning and coordinating the development and implementation of human services programs; and a bachelor’s degree in business or public administration;

or

• An equivalent combination of experience and training that includes two years of professional experience planning and coordinating the development and implementation of human services programs.
