FORMS DESIGNER 2
DEFINITION: This is an advanced working level or supervisory specialized work in designing forms to gather, alter, or disseminate pertinent information.
An employee in this class is responsible, under the supervision of the department's management methods staff, for coordinating a forms design and forms control program consistent with the operational needs of an agency. Work involves working with management analysts and program personnel in gathering and verifying organizational and procedural informational, establishing the objectives of a forms design and control program, and evaluating each request for forms design work to determine need and proper method of developing or designing a new form, revising an existing form, or consolidating forms. Work includes administering and maintaining a forms control program. Work is characterized by the variety of programs for which forms must be designed and the high degree of independence involved in determining how a forms request will be finalized. Supervision may be exercised over a small technical or clerical staff. Supervision is received from a Management Analyst who reviews work for effectiveness and adherence to modern forms design principles and established standards.
EXAMPLES OF WORK PERFORMED: Coordinates, under the supervision of a management analyst, a form design and forms control program consistent with the operational needs of a department or agency.
Supervises s small technical or clerical staff engaged in various phases of a forms design program.
Works with management analysts and program personnel in gathering and verifying organizational and procedural information.
Evaluates each request for forms design work to determine need and proper method of establishment either by designing a new form, revising an existing form, or consolidating forms.
Graphically prepares forms at related materials for duplication.
Works from rough draft or verbal information and applies modern design principles in developing forms.
Plans the size of forms, arrangement of items, and space for information and clarity of captions and instructions.
Develops numbering systems for forms identification, and assigns numbers to individual forms.
Maintains a file of forms masters for reference purpose.
Prepares mechanicals for printing or for use by a composition typist.
Selects the type font used on forms or related materials.
Writes specifications for procurement and reproduction of forms by the most economical and appropriate method.
Maintains records required to document and control the origin, need, and use of all forms in an agency.
Develops and maintains functional files to assist analysts in forms analysis.
Approves proofs supplied by commercial printers.
Performs related work as required.
REQUIRED KNOWLEDGES, SKILLS, AND ABILITIES: Thorough knowledge of the principles of modern forms design.
Thorough knowledge of the characteristics of writing machines used to complete forms.
Thorough knowledge of the differences in purpose and use of construction features such as paper, ink, carbon, punching, perforating, padding, snapouts, and fanfolds.
Thorough knowledge of the type size and spacing requirements of composition machines.
Considerable knowledge of Business English, spelling, and arithmetic.
Considerable knowledge of the different methods of duplicating and reproducing forms and related materials.
Considerable knowledge of departmental organizations and programs.
Considerable knowledge of the basic principles of a forms control program.
Skill in the use of tools, equipment, and materials used in forms design work.
Ability to plan, supervise, and train technical and clerical personnel.
Ability to follow complex oral and written instructions.
Ability to express ideas clearly and concisely, orally and in writing.
Ability to establish and maintain effective working relationships with program personnel, subordinates, and the general public.
MINIMUM EXPERIENCE AND TRAINING: One year as a Forms Designer 1;
or
Five years of clerical experience, one year of which must have included work in management methods, graphic arts, duplicating, or any closely related area, and graduation from high school supplemented by a course in basic forms design; or any equivalent combination of experience and training.
