JOB TITLE: AGING CARE MANAGER 3

SERIES NATURE OF WORK: The Aging Care Manager job series describes professional work providing protective services, social services, or referrals for other services for older adults.

DEFINITION: This is advanced professional work investigating reports of abuse, neglect, exploitation, and/or abandonment, developing service plans, and managing specialized services for older adults. An employee in this job evaluates the medical, social, and psychological needs of older adults, investigates allegations of elder abuse, and offers services to detect, reduce, correct, or eliminate the risks associated with all types of abuse identified during investigations. Work involves acting to ensure the well-being of older adults, as well as their living situation, financial circumstances, personal safety, and quality of life. Work includes conducting investigations of alleged abuse, exploitation, or neglect; interpreting and applying protective services legislation, regulations, policies and operating procedures; implementing actions to protect vulnerable clients; recommending court interventions when appropriate, such as guardianships and involuntary commitments; and documenting all actions and services provided. Work also includes coordinating plans with other agencies to refer clients for additional services, and assisting older adults in utilizing available services to meet their needs. Work is performed independently in accordance with statutory requirements and timeframes; and is reviewed by an administrative supervisor through conferences, reports, and assessment of outcomes.

• Provides specialized care management services to clients and members of their families to improve their quality of life.

• Provides protective services to older adults who are victims of abuse, neglect, exploitation, or abandonment.

• Serves as a liaison to other social service agencies, hospitals, clinics, courts, and community resources to assist clients in utilizing these resources to meet their needs.

• Consults with professional staff in other program areas to discuss assessments, diagnoses, and plans of treatment or support.

• Develops plans to stabilize clients in their current residence or to arrange for improved daily living situations.

• Testifies in court concerning various aspects of assigned cases.

• Arranges for a guardian or a representative payee to protect the clients financial assets.

• Performs the full range of supervisory duties.

• Employees in this job may participate in the performance of subordinates’ work consistent with operational or organizational requirements.

• Performs related work as required.

ENTRY LEVEL KNOWLEDGES, SKILLS, AND ABILITIES:

• Knowledge of the principles and methods of social casework.

• Knowledge of laws, regulations, and policies governing protective and human service programs.

• Knowledge of current social, economic, and health issues that impact the aging population.

• Knowledge of individual and group behavior within the aging population.

• Ability to plan and organize work and set priorities.

• Ability to prepare adequate records and reports related to case management.

• Ability to maintain a client caseload in an effective and timely manner.

• Ability to work with adults who are physically, mentally, or emotionally disabled, or economically disadvantaged.

• Ability to apply critical thinking skills to identify and resolve problems.

• Ability to communicate effectively in writing.

• Ability to communicate effectively orally.

• Ability to establish and maintain effective working relationships.

MINIMUM EXPERIENCE AND TRAINING: (NOTE: Based on the Entry Level Knowledges, Skills, and Abilities)

• One year as an Aging Care Manager 2 or Aging Program Assessor;

or

• One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;

or

• An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
