SERIES NATURE OF WORK: The Aging Care Manager job series describes professional work providing social services or referrals for older adults.

DEFINITION: This is beginning-level professional work managing care services for older adults. 

An employee in this job participates in formal and on-the-job training in the methods, procedures, policies, and regulations governing Area Agency 
on Aging programs. Work involves completing increasingly difficult assignments with greater independence as training progresses and program knowledge
and skills are attained. Assignments include attending training classes, individual and group conferences, and other professional development activities;
reading and evaluating client records and preparing related reports; researching social services and community resources available to older adults; and
assisting clients in utilizing these services and resources to improve their quality of life. Work is reviewed in progress and upon completion for adherence
to training objectives, policies, and procedures. EXAMPLES OF WORK: (NOTE: The examples of work are representative of the work, but every position classified to this job may not perform all examples of work listed.
Conversely, this is not an all-inclusive list of work examples.) * Participates in professional development and on-the-job training activities. * Assists clients in developing or maintaining personal autonomy and identifying supports to address their social, medical, and emotional needs. * Provides referrals and assistance to clients for social services and community resources in order to meet identified needs; conducts follow-up reviews as necessary. * Prepares case histories and reports outlining client information and services rendered. * Performs the full range of supervisory duties. * Employees in this job may participate in the performance of subordinates’ work consistent with operational or organizational requirements. * Performs related work as required. ENTRY LEVEL KNOWLEDGES, SKILLS, AND ABILITIES: * Knowledge of the principles and methods of social casework. * Knowledge of current social, economic, and health issues that impact the aging population. * Knowledge of individual and group behavior. * Ability to plan and organize work.
* Ability to apply critical thinking skills to identify and resolve problems. * Ability to communicate effectively in writing. * Ability to communicate effectively orally. * Ability to establish and maintain effective working relationships. MINIMUM EXPERIENCE AND TRAINING: (NOTE: Based on the Entry Level Knowledges, Skills, and Abilities) * Three years as an Aging Case Aide 2; or * A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or * An equivalent combination of experience and training.
