AGING CASE AIDE 2
Definition: This is advanced public contact work in an Area Agency on Aging assisting senior citizens obtain a wide range of services. An employee in this class assists professional and technical social services personnel in providing information and referral activities to senior citizens to help them better understand the community resources available to meet their individual needs. Work requires that the employe have a detailed working knowledge of the physical, cultural, and social environment of the geographical area or program and an understanding of the technical procedures and practices involved in performing a brief assessment of individual situations. The employe will provide advice, through direct and continuous personal contact, to the clients to help resolve personal problems. Work is performed independently, although guidelines are provided to the employe. Supervision is provided by a professional superior and work is evaluated by the results obtained.
Examples of Work: Searches for isolated older persons in the county through outreach efforts and/or referrals. Interviews applicants and completes in-home assessments of elderly clients to determine appropriate service delivery.
Visits elderly clients in their homes to provide information regarding services for which they are eligible.
Provides detailed explanations to clients relating to the procedural aspects of obtaining available services.
Interviews clients to discuss problems in obtaining services. Refers difficult problems to a caseworker or casework supervisor.
Identifies through use of assessment devices the needs of clients on less complex assessments and reassessments and counsels them in trying to alleviate problems.
Assists the caseworker in duties related to domiciliary care, protective services and other AAA programs to include keeping records, filling out forms and compiling reports.
Accompanies clients to various appointments to assist them in obtaining services.
Conducts follow-up and reassessment of clients to determine current status.
Participants in in-service and out-service training programs designed to provide knowledge and educational requirements for future promotion.
Required Knowledges, Skills and Abilities: Knowledge of the physical, cultural and social aspects of the assigned community.
Ability to obtain, evaluate and analyze information from interviews, reports, records and other type of sources of information; and to provide information to clients and other type of personnel.
Ability to communicate effectively, both orally and in writing.
Ability to establish and maintain positive working relationships.
Ability to learn about Area Agency on Aging Programs and the services of other community agencies.
Ability to effectively communicate verbally with older persons.
Ability to read, write and perform simple arithmetic computations.
Ability to follow simple oral and written instructions.
Ability to express ideas clearly and persuasively to elderly clients.
Minimum Experience and Training: One year as an Aging Case Aide 1;
or
Two years of public contact work, including one year in a human services program;
or
Any equivalent combination of experience and training.
