DEFINITION: This is advanced professional care coordination work through a County Area Agency on Aging, assisting participants in obtaining managed long term services and
supports.
Employees in this job lead the Person-Centered Planning (PCP) process and oversee the implementation of PCP’s. Work involves identifying, coordinating, and assisting
participants to gain access to needed Long Term Services and Supports (LTSS), state plan services, and medical, social, housing, educational, and other services and supports
by providing information to participants, facilitating access, and locating, coordinating, and monitoring needed services. Work also includes informing participants about
required needs assessments; the PCP planning process; service alternatives; service delivery options, including opportunities for participant-direction, roles, rights, risks,
and responsibilities; and assisting with fair hearing requests.
DISTINGUISHING CHARACTERISTICS:
• Work is differentiated from the Aging Care Manager 2 job based on the requirement for increased complexity and authority in directing participants’ access to all
necessary covered services and Medicare, Behavioral Health (BH), and other services.
• Work is differentiated from the Aging Care Manager 3 job based on the lack of protective services work.
EXAMPLES OF WORK: (NOTE: The examples of work are representative of the work, but every position classified to this job may not perform all examples of work listed. Conversely,

• Collects information, including participant preferences, strengths, and goals and informs the PCP.
• Assists participants with the development and management of their PCP.
• Coordinates efforts and prompts participants to complete necessary activities in order to maintain LTSS eligibility.
• Explores coverage of services to address participant-identified needs through other sources, including services provided under the state plan, Medicare or private insurance,
or other community resources.
• Coordinates with other individuals and entities, including facilities, home and community-based providers, referral sources, medical professionals, and informal supports to
provide for seamless coordination between physical, behavioral, and support services.
• Performs related work as required.
ENTRY LEVEL KNOWLEDGES, SKILLS, AND ABILITIES:
• Knowledge of casework principles and methods.
• Knowledge of the use and functionality of Microsoft Office Suite software.
• Knowledge of the use and functionality of the internet.
• Knowledge of the availability of LTSS for the benefit of participants.
• Knowledge of current social, economic, and health problems and resources as they relate to participants.
• Knowledge of the basic principles and methods of program interpretation and community organization.
• Knowledge of individual and group behavior and ways of working effectively with participants who have social, economic, emotional, or health problems.
• Ability to develop care plans for participants.
• Ability to establish and maintain effective working relationships.
• Ability to prepare records and reports in an effective manner.
• Ability to maintain a caseload of participants in an effective manner.
• Ability to communicate effectively orally.
• Ability to communicate effectively in writing.
• Ability to interpret laws and regulations.
• Ability to aid participants to grow in the constructive use of their potential in adjusting to their specific problems.
MINIMUM EXPERIENCE AND TRAINING: (NOTE: Based on the Entry Level Knowledges, Skills, and Abilities):
• Three years of professional social service or health care experience AND holding either a registered nurse (RN) license or having a bachelor’s degree in social work,
psychology, or other related social sciences fields.
SPECIAL REQUIREMENTS:
• Necessary Special Requirement: For individuals qualifying as an R.N., possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State
Board of Nursing; or possession of a non-renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.
• Condition of Employment: Employees possessing non-renewable temporary practice permits must obtain licensure as a Registered Nurse within the one (1) year period as defined
by the Pennsylvania State Board of Nursing.
