Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. The person holding this position is considered to be an essential employee. The Registered Nurse (RN) is responsible for the overall organization, function and performance of nursing personnel on the assigned nursing unit. The RN organizes, directs, promotes and participates in the delivery of appropriate nursing care for patients on the assigned nursing unit and treatment areas. The RN creates and maintains a safe and therapeutic physical environment and milieu. The RN promotes patient health, wellbeing and independence. The RN promotes accurate, thorough and timely communication with other disciplines, nursing staff and nursing management. The RN promotes infection control. The RN maintains professionalism, displays appropriate conduct, complies with all applicable nursing standards of care, and facility and departmental policies and procedures. The RN maintains competency and knowledge in the practice of nursing. The RN performs other duties as assigned. This position is under the immediate direction of the Registered Nurse Supervisor(RNS) reporting to the Nurse Manager 1(NM1) and overseen by the Nursing Department through the office of the Chief Nurse Executive. SUPPORTIVE ACTIVITIES: The RN is responsible for the overall organization, function and performance of nursing personnel on the assigned nursing unit. The duties associated with this include but are not limited to: Directs, instructs, guides and leads professional, paraprofessional and non-professional nursing department personnel on assigned nursing unit. Evaluates competency in the provision of nursing care of professional, paraprofessional and non-professional nursing department personnel on assigned nursing unit within the Registered Nurse’s scope of practice. Monitors paraprofessional and non-professional nursing department personnel for professionalism and quality of work performance on assigned nursing unit. Addresses all professionalism, behavior and job performance issues noted on assigned nursing unit privately, appropriately and professionally with a focus on education and performance improvement. Notifies RNS immediately of any noted professionalism, behavior or job performance achievements or deficiencies. Completes file notes on all interactions with paraprofessionals and non-professional nursing department personnel that address work performance and forwards file notes to nursing supervision for further action and inclusion in the Employee Performance Review. Coordinates with RNS to provide information and documentation encompassing all areas of the Employee Performance Review at the conclusion of the rating period for paraprofessionals and non-professionals on assigned unit. Participates in quality assurance activities as assigned. The RN assesses patient’s physical and mental health status, evaluates needs, identifies patient goals, selects appropriate nursing interventions and completes and/or delegates tasks to paraprofessional and nonprofessional nursing staff within appropriate individual scope of practice to meet patient needs and goals on an ongoing basis on assigned nursing unit. The duties associated with this include but are not limited to: The RN participates in all required aspects of the Interdisciplinary Treatment Team Care Planning Process. The RN assesses the effectiveness of nursing interventions and goal attainment. The RN completes documentation per nursing standards of practice, facility and departmental policy and procedure identifying interventions performed, effectiveness and progress towards goals. The RN ensures completion of all daily professional, paraprofessional and nonprofessional nursing tasks on the assigned nursing units. The RN provides direct care to the patient in the form of assessments, treatments, and assistance with all Activities of Daily Living and Instrumental Activities of Daily Living. The RN assesses/monitors patients for changes in physical and psychological status; any changes are reported to the Prescriber and RNS immediately. The RN administers and documents the administration of medications and treatments in accordance with prescriber orders and all applicable facility and departmental policies and procedures. The RN prepares and accompanies patients to appointments, assists patients onto vans, and secures wheelchair as required. The RN lifts, repositions and transfers patients with assistance and/or utilizing a mechanical lift device. Lifts and carries up to fifty pounds. The RN creates and maintains a safe and therapeutic physical environment and milieu. Duties associated with this include but are not limited to: Completes shift change rounds and hand off report with incoming and outgoing staff. Monitors unit and patient rooms for safety concerns. Completes all required rounds, medication and equipment counts with incoming and outgoing staff. Follows and maintains an understanding of emergency equipment, policies and procedures related to patient safety. Promotes and maintains an interdisciplinary approach in all aspects of patient safety and care. Participates in increased levels of observation and suicide precautions. Responds to all emergencies and provides care per facility and departmental policies and procedures including the utilization of only approved crisis intervention techniques (Mandt). The RN promotes patient health, wellbeing, and independence. The duties associated with this include but are not limited to: Engages with patients on an individual and group basis to provide therapeutic interventions. Provides patient education within the RN Scope of Practice. Encourages patients to participate in self-care activities. Leads structured nursing programs. Acts as a patient advocate in all situations to promote Patient Rights. The RN promotes accurate, thorough and timely communication with other disciplines, nursing staff and nursing management. The duties associated with this include but are not limited to: Completes shift hand off report no later than 1 hour prior to the end of shift and updates as needed. Completes all required documentation in an accurate, thorough, succinct and timely manner utilizing the appropriate format (paper and/or electronic) in accordance with all applicable nursing standards of practice, facility and departmental policies and procedures. Utilizes chain of command and promptly reports all patient and unit concerns. Identify emergent situations requiring rapid response from nursing and other disciplines and utilize the two-way radios/overhead page system. Participates in various committees assigned. The RN maintains professionalism, displays appropriate conduct, complies with all applicable nursing standards of care, and facility and departmental policies and procedures. Duties associated with this include but are not limited to: Demonstrates ethical, professional and respectful conduct with staff, patients and visitors. Follows the policies and procedures of CSSH, the Nursing Department, the Commonwealth of Pennsylvania and all State and Federal regulatory bodies. Reports to their assigned work area ready to work on time for their assigned shift. Reports to the unit RN at onset of shift, signs in on the attendance log at onset of shift and signs out at completion of shift. Leaves assigned unit for assigned breaks, lunch and as directed by Unit RN or Nursing Management. Notifies the unit RN prior to departure and upon return. Adheres to scheduled work hours. Adheres to established policies and procedures for reporting off work. Adheres to DHS #7173, Controlling Absenteeism, Time and Sick Leave Abuse policy and procedures. Commits no acts of insubordination. The RN maintains competency and knowledge in the practice of nursing. The duties associated with include but are not limited to: Maintains current licensure as a Registered Nurse, current BLS certification and current Commonwealth approved crisis management certification. Completes assigned training and in-services. Seeks continuing education opportunities. The duties listed above are not all inclusive, the RN is responsible for the completion of all other duties as assigned. |
