Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This position works in the Therapeutic Recreation Department for Warren State Hospital and has responsibility to develop and conduct appropriate therapeutic recreational, and educational activities for individuals under hospital care in accordance with the overall mission and goals of the hospital. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Develops and conducts therapeutic programming to individuals in treatment in the Therapeutic Recreation program areas, Patient Units, Recovery Center, and designated areas of the hospital grounds. Develops and conducts educational programming for groups in Morning Connections as well as the Recovery Center. Provides 1:1 support to individuals in treatment when indicated. Provide input regarding assessment of individuals for performance and progress; adapt appropriate learning methods and techniques used to assist patients to meet individual functioning levels and treatment team goals; complete assessment documentation and documents progress in weekly/monthly progress notes/Recovery Center notes to monitor activity/functioning levels and progress of individual towards treatment goals. Complete Admission Assessments, and Annual Reassessments, communicating the progress of the patients ensuring the standards set forth by Medicare and Medical Assistance are met. Evaluates patients for inclusion into the Therapeutic Recreation Programs by interviewing, assessing patients, and consulting with treatment team staff in order to determine patient’s skill levels and appropriateness of T.R. activities for them. Assigns individuals to therapeutic recreational activities including various areas such as art, music, social games, crafts, creative writing, leisure education as well as various sports and physical activities. Offer input to Therapeutic Recreation Supervisor for recommended activities based on assessment outcomes, patient interest surveys, and actual needs, as well as reviews current programs for improvement. Records and enters monthly statistics through the electronic patient progress system (EPPS). May attend Interdisciplinary Treatment Team meetings, accept referrals from treatment team and complete accompanying documentation. Provide input toward long and short term goals and develop methodologies to reach goals set by the Treatment Team. Requests and maintains equipment and supplies for T.R. program area by checking equipment, inventorying supplies, repairing and replacing equipment and materials as needed in order to insure that equipment is safe, operational and on hand for patient groups. Performs area maintenance, and seasonal upkeep on all Therapeutic Recreation programming areas. |
