Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The Certified Registered Nurse Practitioner of Behavioral Health or Psychiatric Mental Health (CRNP-BH or PMH) is an independent licensed advanced nursing professional providing psychiatric behavioral/mental health treatment of individuals with severe and persistent mental illness at an inpatient psychiatric Commonwealth facility. The CRNP-BH/PMH is responsible for managing psychiatric problems and coordinating care for the psychiatric in-patient in accordance with Warren State Hospital Policies, State & Federal rules and regulations and the nurse practitioner standards of care (assessment of psychiatric status, diagnosis, development of plan of treatment, implementation of treatment plan, and evaluation of patient psychiatric status). Clinical management is conducted in collaboration with other health care team members and under the collaboration/supervision of the Chief of Psychiatry. Decisions regarding patient care are made within the scope of CRNP and in accordance with WSH Policies and Procedures.

Description of Duties:

Service to include responsibility in patient care and treatment, which may include, but is not limited to:

Applying knowledge of mental illness, signs, symptoms, treatment, and social implications.

Psychiatric assessments/diagnoses - Functioning independently to perform age-appropriate psychiatric evaluations for complex and chronically ill psychiatric in-patient patients.
Admission notes and evaluations - Being responsible for a comprehensive evaluation of assigned patients, including but not limited to, presenting illness, current and history of self-injurious behaviors, aggression, substance use, trauma, hospitalizations and treatment, social and developmental history.

Identifying factors that may affect mental health such as family, trauma, genetics, substance use, spiritual influences and/or environmental factors.

Applying knowledge of the evidence-based theories, principles, practices, and techniques used in the field of mental health, and the relationships between diagnostic findings and therapeutic methodologies.

Applying knowledge of psychiatric and mental health principles to individuals and groups.

Examining/interviewing patients and establishing appropriate diagnoses via the Diagnostic and Statistical Manual of Mental Disorders (DSM).

Prescribing psychotropic and related medications designed to alleviate the symptoms and dysfunction associated with the above diagnoses as well as assessing possible correlated adverse effects.

Consulting with collaborating/supervising psychiatrist whenever the clinical complexity indicates.

Preparing discharge summaries.

Treatment Team functions - Attending and collaborating care as a member of Treatment Team meetings to assist in the development of the ITP and justifying the continuance or modification of the treatment plan. Identifying, monitoring, and measuring clinical/behavioral outcomes to determine the effectiveness of the plan of care.

Ensuring active treatment is occurring on assigned units. Monitoring the patient treatment by making unit rounds daily.

Ensuring that required documentation in the chart is consistent with DHS/OMHSAS regulations, Warren State Hospital requirements, professional nursing, and CMS standards.
Documenting psychiatric care on each newly admitted patient at least weekly and in a manner consistent with current documentation standards.

Collaborating with Psychiatrist to prepare commitment/recommitment petitions and hearings, financial/medical competency evaluations and hearings, and other legal proceedings, as required.

Managing performance improvement responsibilities as assigned.

Providing consultation, education, and advice in the field of Psychiatric Nursing to members of the Treatment Team, including nurses and other clinical staff and allied hospital personnel, agencies, relatives, and other interested persons on the care, treatment, and prognosis of patients.

Ordering, reviewing, and interpreting laboratory reports, test results, x-ray reports, other diagnostic test reports, physical and neurological examinations, consultation reports, and other medical/psychiatric reports in order to alert staff to any unusual circumstances.

Family intervention as dictated by clinical requirements - Collaborating with patients, family, interdisciplinary team members and the psychiatrist supervisor in order to coordinate the patient’s care and on-going disposition planning.

Attending and participating in Community Support Plan (CSP) meetings.

Maintaining a therapeutic relationship with patients, staff and families to promote positive clinical outcomes.

Being available to attend all medical and psychiatric emergencies during the hours of 8:00am to 4:00pm, Monday through Friday.

Responding to medical and psychiatric emergencies, during which the CRNP-BH/PMH will direct psychiatric emergencies, directing verbal de-escalation, prescribing medication as appropriate while following emergency procedures.

Being available to cover other units during normal work hours with no increase in hourly rate, as assigned, in cases where the attending psychiatrist is off duty and according to the number of primary psychiatric staff.

Completing billing and other forms as appropriate.

Participating in discharge planning and communication with the base service units.

Performing other related duties as required.

Attending medical/psychiatric staff meetings.

Responsibilities and duties will be governed and updated by Office of Mental Health and Substance Abuse Service, Department of Human Services, Warren State Hospital, and other administrative and regulatory agencies’ bulletins and directives.

May chair or be a member of any hospital/medical staff committee or task force.

May participate in voluntary on-call coverage.

In all cases, timeliness of responsibilities will be determined by Warren State Hospital.

Clinical management is conducted under the general collaboration/supervision of the Chief of Psychiatry. Administrative Management is conducted under the general supervision of the Chief Medical Officer.

1. Graduate of an accredited School of Nursing (NLN - National League of Nursing) approved program and a graduate of an accredited Nurse Practitioner Program

2. Current valid/unrestricted Pennsylvania Registered Nurse license

3. Verification of a Pennsylvania state Registered Nurse license to practice as a certified Registered Nurse Practitioner that is active and in good standing, with no restrictions or adverse actions taken against it by the State Board of Nursing.

4. Verification of Board Certification in Nurse Practitioner of Specialty area of Adult Psychiatric/Mental Health as issued by PA State Board of Nursing

5. Current professional CPR BLS completion

6. Listing of Continuing Education course, including course title, date(s) attended, and number of continuing education hours earned for the current licensure period
