The incumbent in this position functions as the Facility Safety Manager (FSM) of the Safety Department and is responsible for planning, developing, implementing and controlling the Safety/Security programs at Clarks Summit State Hospital to include supervising and maintaining the Communication Center, fire prevention and suppression, disaster planning, emergency operations, accident investigations, risk management, safety inspections, security investigations, and traffic control functions. The FSM is an essential employee under the direct supervision of the Chief Operating Officer. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Establishes and enforces Clarks Summit State Hospital’s fire/safety regulations, conducts periodic reviews of all departments to ensure compliance with all fire safety and patient safety regulations, and participates as an active member of designated committees and investigates all accidents/injuries. Plans for, coordinates, implements and directs the Safety program by developing plans to meet Environment of Care Standards, investigating accidents, inspecting areas, writing policies and procedures, conducting drills, maintaining records, submitting budget projections, preparing reports, Right to Know Program, etc., in order to provide a safe environment for all patients, personnel, visitors in accordance with federal, state, Medicare, Medicaid, Department of Health (DOH), and Department of Human Services (DHS) regulations. Supervises the Fire Marshal and fire preventative activities, such as fire drills and inspections; fire hydrant flow testing, fire detection and suppression system (sprinkler, detector) testing and maintenance; maintenance/inspection of fire extinguishers; purchases of fire equipment/supplies. Establishes emergency operations procedures for disasters, fire, flood, bomb threat, work stoppage, elopements, workplace violence, etc., by developing operating procedures for all types of emergencies; training staff to function during emergencies; coordinating and sharing services with other agencies during emergencies; conducting annual rehearsals for each type of emergency; updating emergency supplies and equipment as necessary in order to meet any/all types of emergencies which may arise at Clarks Summit State Hospital or locally where the hospital should be a participant. Plans, coordinates, and directs security-related activities, theft control, access control and parking. Establishes regulations and procedures to conform to local and state standards of security. Directs and supervises the Security program by reviewing shift task assignments, reviewing Security logs, spot-checking Security performance, reviewing policies and procedures, submitting budget projections, reviewing schedules, insuring training is conducted, etc., to provide a secure environment for all patients, personnel, and visitors in accordance with all regulations and promulgations. Directs and supervises Communication Center and reception area, including the scheduling of time, completion of progress reviews and evaluations, and implementation of related policies. Ensures the safe and reliable operation of the hospital-wide building entry access system and the video recording system for common areas in all areas of the hospital. Analyzes data on Security/Safety/Fire Safety operations and leads/assists work groups to develop procedures etc., to improve organizational performance. Maintains an accident and injury reporting and recording system from which analysis will indicate casual factors, trends, hazards, needs for revised work procedure, equipment modification or further training. Trains all managers and supervisors in accident investigations and reporting procedures; reviews injury reports for both patients and personnel, and if necessary, conducts further investigations. Supervises mandatory in-service fire/safety training for all staff by developing fire/safety training courses which address issues found at the site; conducts annual mandatory training for all Clarks Summit State Hospital’s staff; conducts quarterly in-service training for all security staff covering all aspects of security and safety duties to reduce accidents/injuries by training all staff in all aspects of safety and fire prevention. Works with Staff Development and department directors to ensure all personnel are familiar with the Safety, Security, Hazardous Material and Waste Management, Emergency Preparedness, Life Safety, Medical Equipment Management, and Utilities Management plans that make up the Environment of Care Standard. Responsible for the hospital meeting all Life Safety Code and follows federal and state regulations concerning hazardous waste, infectious waste, and environmental matters. Functions as the lead Local Violence Assessment Team (LVAT) coordinator, and an administrative investigation investigator on rotational basis. Prepares written reports for documentation of LVAT investigations, issues cases to LVAT members and ensuring reports are properly completed and filed with the Office of Administration, Workplace Violence Office. Serves as the Right to Know Coordinator and follows the Department of Human Services (DHS) Safety Manual and the AIPP Program Manual P.6. Ensures that specific Right to Know training for personnel working with hazardous substances is conducted and properly documented. Training must be provided to each person working with hazardous substances when a new substance is introduced as well as annually. Maintains Material Safety Data Sheets (MSDS) on hazardous substances; completes and posts hazardous substance survey forms; posts personnel workplace notices; ensures training is provided to personnel; ensures confidentiality of health and exposure records is maintained. Serves as the Coordinator for the Hazardous Waste and Hazardous Material Program and medical monitoring for the Asbestos Safety Program; Respiratory Protection Program; fit testing, Medical Surveillance Program for Asbestos Workers and Respirator Users; Radon Program; Emergency Communications; Photo Identification Tags; Key and Lock Control Program; product Recall/Product Safety Program and the Emergency Behavioral Health (EBH) responder. Manages, develops, coordinates, and implements a facility safety program, including training and education, accident, investigation, enforcement of safety rules and regulations, safety inspections, establishment of appropriate records and reporting system, and the integration of appropriate medical services into the safety program. Maintains, revises, and updates the hospital’s Emergency Operations and Safety Manual as part of the Safety Committee. Plans, develops, and coordinates the implementation of new or revised safety, fire prevention and security training programs, and ensures that all subordinates and new employees are familiarized with policies and procedures as they relate to their job functions; and conducts refresher courses as required. Coordinates hospital information with various external emergency service agencies. Acts as hospital liaison with community emergency management organizations, fire companies, and federal, state and local law enforcement agencies. Investigates accidents involving injuries or property damage to determine causal factors and recommends measures to prevent or minimize recurrences. Receives training and conducts investigative training as assigned. Reviews plans and specifications for the purchase or construction of new equipment and facilities to ensure that departmental safety requirements are met. Conducts employee interviews and makes recommendations for hiring. Presents workshops on the development of various aspects of the facility safety program to functional area supervisors and line supervisors. Conducts, organizes, and prepares written evaluations of safety training courses. Conducts required drills and tests and maintains necessary documentation for reviews or surveys. Detects occupational, industrial, fire and safety/security hazards, and devises corrective measures. Supervises facility personnel assigned to the fire prevention and security functions. Administers collective bargaining agreements; approves leaves and reviews work performance; counsels and takes progressive disciplinary action; monitors leave usage of department. Establishes and implements policies and procedures for all Fire/Safety/Security/Communication Center functions. Plans, assigns, directs, coordinates, and provides written evaluations of the work of the Fire/Safety/Security/Communication Center personnel engaged in hospital-wide operations. Prepares or supervises the preparation of reports relating to accident injury data, fire and prevention data, and security operations as required. Gathers, correlates, and analyzes facts and arrives at sound conclusions. Attends and participates in workshops and seminars pertinent to various aspects of facility safety management. Instructs and leads group discussions and conferences on safety. Establishes and maintains effective working relationships with hospital, agency, and contracted personnel. Communicates effectively, orally and in writing. All reports are complete, usable, and meet deadline dates and times. Maintains clear, complete, and accurate records. Obtains and maintains Crisis Response/Intervention certification through vendor approved by the Commonwealth. Performs other related duties as required. |
