Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. The employee develops, implements and provides social services, counseling and case management services to forensic patients with a mental illness who have been placed through the criminal justice or correctional system for diagnosis, evaluation and intensive treatment. Programs are designed to enhance the patients’ social functioning and to help them attain a more satisfactory social and emotional adjustment. Work involves interacting with forensic patients, family members, guardians, facility/unit staff, and staff from external agencies to plan, provide and/or coordinate needed forensic patient social services. Work includes obtaining social, medical, financial and commitment information for use in assessing forensic patient social services needs; providing counseling services; participating with other facility staff in forensic patient care and treatment and discharge planning for an assigned caseload of forensic patients. An important aspect of the work is the responsibility for the care, custody and control of forensic patients with a mental disability while participating in social services programs. Work is performed in accordance with applicable rules, regulations, policies and procedures under the supervision of the Forensic Social Work Supervisor. Work is reviewed through conferences, reports and results achieved in meeting the facility/unit social service program standards, goals and objectives. Interviews forensic patients, their family members, external agency representatives and others to obtain, update and record information regarding forensic patient’s physical, mental, emotional, economic and social status to assess their psychosocial strengths, weaknesses and social service needs. Assesses forensic patients social service needs; provides education to the patients, their families and other interested parties, conducts individual and group therapy utilizing various treatment modalities in order to assist the patients in recognizing their problems and assist the forensic patient in modifying and improving personal and social function and behavior. Collaborates with other facility/forensic psychiatric unit staff in multi-disciplinary treatment team meetings by discussing forensic patient psychosocial assessments and participates in the development and implementation of each assigned forensic patients comprehensive individual care and treatment plan. Records on assigned forensic patients their response/progress in meeting social service treatment program goals and objectives. Maintains communication with assigned forensic patients, their family members and other interested parties to advise them of the status of patient’s progress and encourages their support and input. Writes social service discharge/transfer plans summarizing forensic patients’ dispositions and/or the need for continued aftercare or follow-up services upon discharge, as appropriate. Maintains a record to assure all patients are under a current court commitment. Completes recommitment petitions, attends recommitment hearings and records outcome by updating the patient’s chart. Provides for or actively assists in providing care, custody and control of patients assigned during social service programs and/or on the unit. Performs other related duties and special projects as assigned. The purpose and standards will be given at the time of each assignment. Receive and maintain certification in basic crisis intervention techniques, currently Mandt. Demonstrate competencies in application of techniques and maintain certification in Advance Mandt Training. Receive and maintain certification and competency in CPR. |
