Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Under the general supervision of a professional supervisor, is responsible for providing a full range of Pre-ETS and vocational rehabilitation services to persons unable to hear or understand speech, even with amplification, to assist them in obtaining gainful employment. Develops and maintains referral sources and liaison relationship with schools and community site including Will cover Pre-ETS and Adult/VR referrals from the unit coverage based on zipcode assignment. Counselor maintains personal contact to assure that potential vocational rehabilitation customers are identified. Obtains preliminary information pertinent to the referral of persons with disabilities for vocational rehabilitation services. Conducts a comprehensive face-to face interview with the applicant to gather pertinent information regarding the nature of the disabling condition, educational and employment history, psycho-social factors, and other data needed to initiate the vocational rehabilitation process. Explains eligibility requirements, appeal rights, and determines next steps with the applicant and other appropriate parties. On the basis of medical, psycho-social,and customer information, determines the applicant's potential for vocational rehabilitation. Guided by legislation, regulations, and agency policy, utilizes professional judgment, consultation as appropriate with related professionals, and the customer's participation. Collaborates with supervisor to arrange for expenditures associated with medical, psychological, psychiatric, physical or other evaluations as part of determining potential for vocational rehabilitation. Coordinates the delivery of these services through completion. Evaluates the information obtained to ascertain the applicant's potential for rehabilitation and eligibility. In consultation with the customer and other appropriate interested parties, determines a vocational goal and develops and approves a mutually agreed upon Individualized Plan for Employment to achieve the established vocational goal. Identifies customer and agency responsibilities, collaborates with supervisor to arrange for expenditures associated with determining eligibility and/or providing vocational rehabilitation services. Coordinates the delivery of planned vocational rehabilitation services and provides counseling, guidance and placement assistance as needed to assist the customer in achieving the established vocational goal. Maintains, evaluates, and documents customer process through the vocational rehabilitation process and makes appropriate rehabilitation plan amendments to enhance the opportunity for customer success. Follows up with customers on a regular basis to review progress. Prepares case records and reports in accordance with current policy and procedures. Follows up with customers to determine the appropriateness of case closure. Provides professional vocational rehabilitation services and maintains liaison with community services providers to assure that service programs address customer needs. This includes but is not limited to Careerlink services. As necessary, provides technical and professional guidance and assistance concerning Agency programs, procedures and services. Contacts employers to enhance placement opportunities for individuals with disabilities. Performs related work as required. Accesses and updates participant records using a web-based application. Utilizes a web-based application to access participants referred for services. Make referrals for appropriate services through the CWDS. (Commonwealth Workforce Development System) Enters detailed record information directly into CWDS by personal or phone contact with potential participants. Enters case notes into CWDS in a timely manner so they can be viewed/shared. Enters cost service information into a service authorization through CWDS. Reviews and customizes online letter used to inform participants about services authorized. Accesses open encumbrances online. Views invoices and reimbursements online and cancels if necessary. Determines what customer equipment needs to be tagged and enters this information into the on line service authorization. Generates correspondence as needed within CWDS. Completes all applicable customer information within CWDS to complete and maintain the electronic case file. |

Counselor makes decisions determining eligibility of consumers for vocational rehabilitation services. Makes decisions to arrange expenditures associated with consumers IPE. Makes decisions pertaining to the appropriateness of case closure. Decisions referred to others would include any conflicts that are not able to be resolved on an individual basis between the counselor and consumer. These decisions would be referred to the immediate supervisor for assistance. |
