Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This position works in a Regional Forensic Psychiatric Center (RFPC) providing services to mentally ill patients referred by the criminal justice system. This position is a lead worker over lower level forensic security employees and provides non-professional nursing care for patients with varying degrees of emotional and physical disabilities with responsibility for maintaining a therapeutic, secure environment. This position may also serve as a trainer, classroom trainer, and On the Job Trainer.

Description of Duties:

1. The Forensic Security Employee (FSE) 2 works under direction from the Forensic Registered Nurse (FRN) for purposes of clinical direction, performance evaluation and overall guidance of work performed on assigned unit(s) during the shift. The Forensic Registered Nurse Supervisor (FRNS) manages multiple units and is to be consulted in emergent situations or in the absence of the FRN. The FSE 2 provides non-professional nursing and attendant patient care in accordance with patients’ Comprehensive Individualized Treatment Plan (CITP) contributing to the quality of the therapeutic environment. The FRN provides safety, security and clinical oversight.

2. This position has responsibility to conduct on-the-job training for FSE Trainee staff and provide lead work for the daily tasks of the FSE 1 staff to insure daily routines of the unit flow smoothly. On-the-job training for FSE Trainees following the FSE training program includes, but is not limited to, teaching security procedures, ward and unit routines, escort in and out of the unit, searches of person/environment, visit procedures, control booth functions, use of sally ports, control of keys, use of walkie-talkies and hostage and escape drills, and all other information contained in the training modules.

3. Conduct training programs explaining the tasks to be done, demonstrating tasks, observing and evaluating trainee performance of tasks in accordance with job expectations with emphasis on minimizing safety and security risks in accordance with appropriate policy and procedure; assisting the trainee to correct substandard performance and documenting trainee accomplishments in the trainee competency manual on a daily basis; reports trainee progress to FRN, FRNS and Nurse Manager.

4. Responsible for observation and review of FSE1 and Trainee routine job tasks such as face-to-face patient accountability, environmental inspections, timely, accurate completion of staff assignment sheets, key control, control booth duties, completing searches and maintaining compliance with policy and regulation to ensure a safe, secure environment for visitors, staff and patients. Provides training to trainees and FSE1s on all established policies and procedures in accordance with DHS, OMHSAS, RFPC rules and regulations and ensuring that all patient rights are maintained.

5. Provide internal safety and security procedure training to the State Hospital staff and community liaisons that perform duties in the Forensic unit, insuring all forensic staff are properly trained on all new security policies or procedures.

6. Provide daily routine care for patients and ensures a safe, secure environment for patients, staff and visitors to the facility with responsibility for continuous monitoring and supervision of patients, living areas and daily activities, particularly during the evaluation and treatment process. Performs simple nursing procedures, i.e., collects urine specimens, takes and records temperature, pulse, respiration, and blood pressure; measures the weight and height of the patients, monitors soaks and provides skin care and basic nursing care as instructed.

7. Work closely with patients to accomplish activities of daily living and self-care skills such as bathing, oral hygiene, hair care, dressing and other personal care needs; provides assistance and guidance based on individual patient needs; incorporates daily routines to meet individual patient treatment plans; actively engages patients in a therapeutic manner, following individualized plans of care and clinical methods developed by treatment team. Provides emotional support to patients offering encouragement through positive interaction in group and one-tone settings; acts as individual patient advocate to assigned patients by serving as a primary contact person, assisting with personal needs and belongings; shows respect to patients at all times and protects the right of confidentiality with patients, coworkers, and any other treatment partners.

8. Participate as part of the interdisciplinary treatment team in development of treatment plans and implementation of interventions, providing feedback regarding patient progress through verbal reports and written progress reports; attends in-service training sessions to supplement knowledge and skills.

9. Complete daily, random environmental and face-to-face rounds of assigned areas including space used for patient programming to ensure that integrity of the forensic center is not compromised; includes rounds to visually account for all patients and document accounts of rounds in accordance with applicable policies and procedures.

10. Complete legible documentation in the patients’ medical record of various patient care components, in correlation with Comprehensive Individual Treatment Plan (CITP), and performance goals as per Nursing/Forensic documentation policies and procedures; other documentation includes incident reports (SI-815), and other pertinent forms completing information relative to changes in patient’s mental and physical condition as noted through observation and interaction.

11. Attend and participate in shift report, taking responsibility for hand off communication, reporting patient status, daily shift assignments communicating clear, concise information to the oncoming FRN before leaving duty and other times as necessary. Leads the staff in ward/shift routine and assists co-workers in adapting to changes in assignment.

12. Work closely and collaboratively with co-workers and all levels of management to make the best decisions to meet the needs of the facility and the patients. Maintains a positive, respectful working environment with coworkers, patients and all others they may come in contact with during the shift.

13. Conduct daily security and Environment of Care rounds i.e. checks doors, windows and furnishings in accordance with applicable policies and procedures reporting any deficiencies immediately; conducts random unannounced searches of patient living areas and secured areas i.e. gym, chapel, OT, etc. in accordance with applicable policies and procedures.

14. Assist with admission, transfer and discharge of patients; escorts patients to and from any area within the building or off grounds as assigned. Escorts staff as needed.

15. Screens visitors for various unauthorized materials including contraband; monitors visits, with eyes on patients and visitors to prevent security risks, observes activity to identify violation of policy i.e., pass contraband, etc. and ensures policy and procedure are followed at all times.

16. Ensures compliance with all established policies and procedures in accordance with DHS, OMHSAS, RFPC rules and regulations; reports or corrects violations immediately; maintains working knowledge of all current Hospital, Nursing and RFPC Policies and Procedures and uses manuals as reference as necessary. Seeks and accepts supervision in unfamiliar situations; recognizes problems and suggests and/or assists in developing solutions.

17. Inspect mail and packages by trans frisking, logging, and inspecting all contents in the presence of the patient in accordance with RFPC procedures.

18. Appropriately utilize all communication devices - including telephone, intercoms, two-way radios, entry systems and closed-circuit television including sally port functions; responsible for control of personal, living area, and other keys, reporting lost items immediately following policy.

19. Implement security, fire, safety, infection control and emergency policy and procedures of the facility as appropriate; recognizes, documents, and determines disposition of contraband in accordance with policy. Communicate security issues to shift FRN/FRNS, submit maintenance work order as needed. Conducts and documents internal security and safety inspections of the RFPC during the assigned shift.

20. Demonstrate appropriate use of currently taught techniques related to crisis management interventions in accordance with state wide curricula to prevent or reduce escalating aggressive behavior in patients and others.

21. Actively prevents escape by maintaining close observation of patients and being aware of their location and activity. Reports any threats of escape or rumors of unrest and participates in planning of prevention techniques. Makes rounds at change of shift and hourly to assess environmental and security risks. Reports any security or environmental issues identified to FRN/FRNS immediately.

22. Provide escort and security procedures for patients escorted into the community for medical treatment or appointments. While in the community the FSE continually visually accounts for all patients in accordance with applicable policies and procedures actively prevents escape by maintaining close observation of patients and being aware of their location and activity. Reports any security or environmental issues identified to FRN/FRNS immediately.

23. Implement Forensic code system for emergency responses and demonstrates appropriate use of code; responds to and assist in emergency situations; performs CPR when appropriate.

24. Inspects all areas of the facility being used by Forensic patients.

25. Provide input and recommendations for correcting security deficiencies or improving overall security of the RFPC. Conduct and document internal security and safety inspections of the RFPC during the assigned shift.

26. Participate in evacuation and disaster drills and adheres to emergency operations plan, demonstrates knowledge of the forensic evacuation procedure.

27. Attend and complete all in-service training and educational programs as assigned to meet competencies i.e. CPR, Abdominal Thrusts, crisis management interventions, AED use, First Aid and etc.; completes self-learning packets.

28. Report safety hazards and malfunctioning equipment to FRN or other management staff; removes any broken furniture or unsafe items from living areas and notify the immediate supervisor; maintains proper care, safety and use of hospital equipment.

29. Participate in infection control, environment of care, and basic housekeeping functions within patient living areas, common areas, and staff designated areas; handles clean and soiled laundry in accordance with established procedures.

30. Perform routine housekeeping duties to maintain environment in accordance with hospital policy. Disposes of trash and garbage properly.

31. Requisitions, inventories, and distributes supplies. Maintains a clean, safe, and orderly supply area.

32. Adheres to DHS policy 7173 regarding time and attendance.

The FSE 2 makes daily work decisions regarding routine care and security procedures based on training, skill, and security in accordance with DHS and facility established policies/procedures, RFPC rules and regulations.

The Forensic Registered Nurse, Forensic Registered Nurse Supervisor, Nurse Manager, CNE, CFE, and/or hospital leadership provide direction regarding clinical, environmental, security, and patient care duties.

For those positions who serve as FSE Trainers:
Person assigned as a Trainer must demonstrate job knowledge and skills while training. Provides training to staff that is relevant to course content and to meet competency in identified subject area. Provide information consistently to other trainers and supervisor regarding course content. Provide information consistently to supervisor regarding performance of those in class. Be knowledgeable in proper techniques and usage of training equipment and remains involved at all times when instructing. Takes initiative to, identify, report and correct trainee understanding. Trainers will provide training 90% of time as scheduled. Organizes tasks, requests feedback on tasks performed and modifies work performance based upon that feedback. Determine the need in writing for the identification of problems or issues that need to be referred to the supervisor for resolution.

Essential Functions
: Provide a list of essential functions for this position. Example: Transports boxes weighing up to 60 pounds.
 
 1. Lead work for FSE 1s and trainees in daily routine and security procedures.
 2. Lift at least 60 pounds i.e. boxes, assisting with lifting and repositioning patients.
 3. Climb three (3) flights of stairs consecutively, kneel, squat, stretch and reach. Walk a continuous half mile while perf. searches of the perimeter.
 4. Demonstrate knowledge of all forensic, local policy and procedures. Demonstrate appropriate oral and written communication skills.
 5. Utilize proper body mechanics to provide patient care, support crisis intervention techniques and respond quickly to emergencies.
 6. Perform CPR and intervene in any crisis situation i.e. medical emergencies, codes, etc.
 7. Maintain a safe and therapeutic environment, fosters respectful relationships with patients, co-workers and others.
 8. Act as a patient advocate assuring that all patient rights are maintained.
 9. Provide factual lead worker observation, documentation, and follow through to achieve corrective action and provide retraining as needed.
 10. Receive and maintain certification in basic crisis intervention techniques, currently MANDT.
