Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. The Quality Assurance/Risk Management Coordinator position supports the State Center’s Quality Management and Risk Management Programs and contributes to the organization’s mission, vision and values that are consistent with the Department of Human Services/Office of Developmental Programs. The Quality Assurance/Risk Management Coordinator will perform their work functions within the Risk Management Department in concert with the QA/RM Director. The Quality Assurance/Risk Management Coordinator will monitor, audit and evaluate the quality of services and supports, assist in preparation for and responses to licensing surveys, participate in the development and improvement of the State Center’s Quality Management and Risk Management Programs, facilitate assigned meetings/committees/focus groups and performance improvement teams, develop reports and provide guidance and technical assistance to interdisciplinary clinical and non-clinical staff. The Quality Assurance/Risk Management Coordinator will coordinate investigations into all HCSIS required incidents. The Quality Assurance/Risk Management Coordinator supervises assigned staff in the Risk Management Department. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 1) Participates in the provision of orientation and continuing education programs to staff, individuals, families and other stakeholders in Quality Management and Risk Management principles, identification of performance measures, performance improvement team process, data collection methodologies, display and analysis of information and generation of reports, including recommendations, action plans and follow-up interventions resulting in operational improvements in efficiency and/or effectiveness. 2) Supports the development of and follow-through on Plans of Correction related to licensing surveys and regulatory compliance. 3) Assign investigations as needed, maintain daily log of all investigations and summarize data on investigations monthly. 4) In conjunction with the QA/RM Director, provide 24 hour consultation regarding incidents that occur. 5) Supervises the work of the Quality Assurance/Risk Management Specialist in supporting the facility in the use of Quality Improvement and Risk Management tools to collect, display, analyze and report information for Quality Management. 6) Works in concert with the QA/RM Director in facilitating and/or supporting the Daily Risk Management Review Meeting and Bi-weekly Risk Management Committee. 7) Reviews all incident reports that occur to ensure proper preventatives have been initiated. 8) Monitors and audits the accurate completion and supporting documentation of each corrective/preventative measure for approximately 190 incidents each month. 9) Supports Selinsgrove Center Incident Closures in (HCSIS) with closures of all optional incidents and Center closure of reportable incidents. 10) Attends and supports the Selinsgrove Center Quality Management/Risk Management Committee and Quality Improvement Council by assisting assigned staff in their report preparation and presentations. 11) Provides guidance and technical assistance to Initiative Leaders, Managers and Supervisors as they apply Quality Management and Risk Management principles. 12) Serves as a Certified Investigator (CI) and monitors and audits the accurate completion and supporting documentation of each CI recommendation. 13) Facilitates monthly workshops for certified investigators, focusing on quality management and enhancement of skills. 14) Is a member of the State Center Certified Investigators Workgroup 15) Participates in Bureau level Quality Management/Risk Management Committees that combine the efforts of State Center staff in using available data and quality management processes to advance the initiatives of the Office of Developmental Programs across the system as assigned. 16) Utilizes computer expertise including a working knowledge of word processing and spreadsheet functions, and database utilization, maintenance and management 17) Maintains and upgrades skills by attending meetings, training sessions and seminars and by reviewing current literature. 18) Reviews and finalizes all investigation in HCSIS. 19) Trains all staff on Abuse and Title 55 during employee orientation and as needed for all employees. 20) Ensures that the peer review process is completed for 10% of all certified investigations per quarter. 21) Conducts monthly certified investigator workshops. |
