Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The employee occupying this Social Worker position is assigned to the Social Service Department and provides services to 100% of that patient population. Services include both direct clinical contact, family/community agency contact, social work consultation to other professional disciplines and the patient's assigned treatment team. In addition, this social worker provides admission assessments when deemed necessary and specific duties as assigned by the Social Work Supervisor and the Director of Social Service. These are indicated in the job standards. This Social Worker is under the direct supervision of a Social Worker Supervisor. This Social Worker I works with a population aged 18 years and older. This position will be working in Greizman (civil), and Renner (civil).

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

Interviews individuals, their family members,
legal guardians, and external agency representatives to obtain, update
and record information regarding individuals' physical, mental, emotional,
economic and social status.

Assesses individual's psychosocial strengths, weaknesses and social
work services needs, and eligibility for services.

Counsels individuals or family members utilizing various treatment
modalities.

Assists individuals or family members in recognizing problem[s] and
exploring methods for adjusting, modifying and improving personal and
social behavior.

Completes psychosocial assessments annually and initially.

Discusses assigned individuals' psychosocial assessments and family
history in multidisciplinary treatment team meetings.

Participates in the development and implementation of each assigned
individual's comprehensive individual care and treatment plan.

Completes individual goal planning for individual plans of care based on DHS/Torrance policy.

Participate in the delivery of active treatment services in the form of groups or individual interventions.

Ensures needed benefits and services are obtained in accordance with
assigned individuals' treatment plans and applicable mandates,
regulations, and policies.

Apply for Medical Assistance, Social Security, and other benefits for the patients.

Provides the full range of social work services for an assigned caseload in
a mental health facility of 20+ patients.

Records, updates and maintains assigned case management files on
individuals' progress in meeting treatment plan goals and objectives.

Documents visits/phone calls with family members and friends and refers any issues to the treatment team for review.

Advises individuals and their family members/legal guardians of progress
status and encourages their support and input.

Completes mental health petitions for involuntary commitment following the mental health procedures act requirements and guidelines.

Responds to correspondence relating to court documents, parental or
relative support, medical consultations, contracted service providers, rate
payments for care and maintenance, and public and private insurance
reimbursements.

Assists patients in legal matters that are prior issues or that have presented during their stay at Torrance.

Confers with facility staff, external agency representatives and family
members on discharge planning.

Initiates the CSP process and coordinates the clinical assessments for patients on an annual basis, when discharge is pending, and as needed.

Writes social services discharge/case closing/transfer plans summarizing
individuals' disposition and the need for continued aftercare or follow-up
services upon discharge.

Works with, visits and inspects Community Based Service Units, service
providers, and social and health care agencies to maximize care and
treatment and aid individuals' transition back to the community or home.

Prepares records and reports in accordance with established policies and
procedures.

Performs related work as required as assigned by the Social Work Supervisor and Social Work Manager.

Attends and participates in legal and mental health hearings to testify.

Attends multi-disciplinary treatment teams weekly as a required discipline of the hospital.

Participates in conference calls related to patient care.

Transports patients to trial visits, home visits, and other social service duties as assigned by the social worker supervisor and social service director.

Completes risk profiles for assigned members of the unit on a weekly basis.

Completes referrals for services in the community and while in the hospital.

Assist patients in withdrawing money from their Torrance Guardian Office account.

Prepares recommendations and schedules active treatment programming in accordance with patient needs and goals for treatment.

Maintains contact with probation officers, judges, Megan's law registry's in accordance with legal parameters.

Notifies supervisor immediately of issues on unit.

Receive and maintain certification in basic crisis intervention techniques, currently Mandt.

Demonstrate competencies in application of techniques and maintain certification in Advance Mandt Training.

Receive and maintain certification and competency in CPR.

Clinical decisions related to patient care such as discharge resource, active treatment can be made independently
Notification of Supervisor/Director should be made related to legal problems and possible contacts with lawyer
Overtime must be approved through the Supervisor or Social Work Manager.
