Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The employee in this position is an advanced professional nurse who performs as a charge nurse by directing and coordinating nursing care delivery on an assigned shift, in multiple living areas. Duties are performed in accordance with established standards of practice, policies/procedures.


Description of Duties:

Team Nursing Responsibilities:
Utilizes the Nursing Process at Selinsgrove Center in order to maintain high-level wellness for each individual. This is done in compliance with Federal, State and in-house regulatory modes. This is accomplished by:
1.Delegating and coordinating subordinate work force to ensure the efficiency in operations.
This is accomplished by:
A. Setting priorities for assigned shift/area: i.e.
(1) assigning breaks and mealtimes with appropriate staff coverage during these times.
(2)delegating nursing unit activities effectively on assigned shift.
Adapting to changes in work assignments or responsibilities to meet the needs at the worksite on assigned shift.
2.Participating in the interdisciplinary process and the development, implementation, evaluation, and modification of the comprehensive Active Treatment Plan.
This is accomplished by:
A. Attending annual reviews for those in assigned caseload, when on duty, and completing an annual nursing assessment packet to be presented at the annual review.
B. Maintaining a cooperative relationship with staff from all disciplines serving the unit’s population in order to maximize communication and ensure the most comprehensive and meaningful habilitation program for each individual.
3.Maintaining open communication by relaying messages, exchanging ideas in a constructive, courteous, effective manner.
This is accomplished by:
A. Acting as a liaison to physician, providing information and reporting observations, as well as assisting with examinations, as needed.
B. Completing rounds with consultants as needed and participating in special health care interventions as indicated.
C. Following procedures established for taking, implementing, and documenting physician’s orders to ensure medical care is being provided as ordered on assigned shift.
4.Ensures effective and efficient nursing care delivery on assigned shift/area.
This is accomplished by:
A. Completing professional nursing functions, as described by Nursing Standards of Practice, for health care needs of individuals in the unit to provide optimum nursing care.
B. Performing related duties according to standards and target dates determined at the time of each assignment.
C. Screening individuals daily for signs and symptoms indicating a need for change in the treatment plan and reports to the appropriate physician on a timely basis.
D. Completing assessment and follow-up of any reported illnesses.
E. Observing, interpreting, recording, and reporting the effects of pharmacological agents and therapeutic treatments.
F. Reviewing results of diagnostic procedures/evaluations and reporting any abnormality to physician.
G. Maintaining nursing responsibilities for assigned caseload in accordance policies and procedures. Applying nursing techniques required for the intensive and continued care and treatment of the individuals in assigned area(s). Completing documentation timely, legibly, and in accordance with established policies and procedures.
H. Familiarizing himself/herself with all DHS and institutional regulations as applicable to his/her professional area.
I. Receiving oral/written report at the beginning of shift and giving oral/written report to oncoming nurse(s); assuring completion of 24 Hour Nursing Report on assigned shift (inclusive of individual acuity rating).
J. Communicating any changes in health status to unit supervisor and providing input on 24 Hour Supervisor/Nursing Report. Making rounds with unit supervisor at lease once every shift to screen for obvious health problems.
5.Work activities may include:
A. Lifting, positioning, and transferring all assigned individuals with the assistance of other employees.
B. Pushing wheelchairs and other adaptive devices on the level and up and down ramps.
C. Performing emergency interventions such as CPR/Abdominal Thrust.
D. Lifting, pulling, pushing, and carrying objects.
E. Initiating safety mechanics (restrictive/therapeutic holds).
F. Initiating and performing emergency evacuations.
The frequency and duration of these activities may vary from unit to unit and shift to shift.
6.Additional work activities include:
A. Understanding and interpreting oral and written instructions.
B.C oping with day-to-day problems and make reasonable decisions regarding individuals served.
C. Meeting all elements of performance standards.
D. May be reassigned to any area of the Center on an as needed basis.
E. May be assigned mandatory overtime when insufficient volunteers are available to meet the minimal staffing requirements.
These duties are essential to all components of the job in order to ensure the health and safety of individuals served.
7.Performing all related duties which are compatible with job description and those which are necessary in an emergency in order to ensure the health and welfare of those individuals who reside at the Center.
8.Maintaining licensure requirement, and current CPR training requirements.
9.Maintain professionalism.
This is accomplished by:
A. Performing work responsibilities creatively, resourcefully, and with the spirit of a “self-starter”.
B. Relating effectively on a verbal and non-verbal basis with all staff and individuals.
C. Identifying strengths and weaknesses in overall performance and trying to grow in areas of personal weakness.
D. Enhancing his/her own professional growth and knowledge through active participation in professional meetings, conference, inservice sessions, and out-service training programs.
10.Fulfilling the following general requirements in relation to work location and hours of work:
A. Emergency QMRP duties for behavioral interventions and post-hospital admissions adhering to the policies/procedures related to this role.
11.Carrying and using 2-way radio as needed.
ESSENTIAL FUNCTIONS:
1. Apply professional nursing practices and methodology for the care and treatment of individuals.
2. Direct and coordinate the delivery of nursing care and treatment for individuals on a single shift.
3. Carry out treatment programs and provide physical nursing care services to individuals.
4. Develop and implement an appropriate treatment regimen in accordance with prescribed medical/psychiatric/treatment program orders.
5. Administer prescribed medications and treatments and observe, interpret, record and report vital signs, symptoms, and reactions to medications and treatments.
6. Position and transfer individuals, some of whom are non-ambulatory, and transfer equipment required for patient care. Lifting of individuals is typically accomplished with the help of another direct care staff person.
7. Communicate effectively/maintain effective and cooperative working relationships.
8. May be reassigned to any area of the Center on an as needed basis.
9. May be assigned mandatory overtime to meet minimal staffing requirements.
10. Prepare and maintain records and reports.

Serves as charge nurse by directing and coordinating nursing care delivery on an assigned shift, in multiple living areas. Independently make decision to report any and all health care practices that are not current best health care practices. Observe for reactions and/or responses to medication and treatments, and report observations.
