The Social Work Supervisor is responsible for the planning, directing, and controlling a full range of social services for the residents of the facility. Incumbent provides supervisory and management leadership and judgment in stimulating, planning for, customer and resident service, positive change, ethical conduct, teamwork, diversity, and inclusiveness to improve services, streamline operations and effectively produce positive results. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 30% Supervise and manage overall operations of the Social Services department involved in providing social services, counseling, and case management services to residents. Directs department operations by establishing departmental policies and procedures, assigning duties and responsibilities, and establishing standards for performance. Ensures residents and their families are informed on resident’s rights, notified, and screened regarding advanced directives and interviewed about voting status. 20% Oversee all administrative/clerical functions related to resident assessments, resident admissions, transfers, and discharges and resident council. Maintain accurate and complete written documentation of the frequency and nature of social services provided to residents to meet facility policy requirements and regulations of accredited agencies. Integrate each resident’s psychosocial assessment with their person-centered plan of care. 15% Participates as a member of the interdisciplinary care plan team by attending meetings and maintaining accurate and complete written documentation. Ensures all charted documentation is informative of the frequency and nature of social services provided to residents to meet BVH and SVH policy requirements and regulations of accredited agencies. Completes assigned sections of the resident assessment instrument on admission, quarterly, annually, and as needed basis. Integrates each resident’s psychosocial assessment with their person-centered plan of care. 15% Functions as the Grievance Coordinator, monitoring and managing the investigation follow through processes in accordance with facility policy and procedures. 10% Counsels’ residents, their families, and others individually or in groups utilizing various treatment modalities such as supportive counseling, grief counseling, family therapy, crisis intervention, behavior modification, etc. to help them recognize the problems and to explore methods for adjusting, modifying, and improving personal and social functioning and behavior. 10% Represents the department on several standing committees of the Home, in addition to periodic administrative meetings. Responsible for promoting both departmental and interdepartmental cooperation by providing program planning and development, staff development, training needs for staff members, and other departments as requested. Maintain positive, constructive working relationships with co-workers; remain aware of the needs and sensitivities of others. Conduct job duties in compliance with established BVH and SVH policies, guidelines, and procedures. Maintain and improve job skills and competencies by participating in and attending staff training programs in order to improve the quality of resident care. Performs work in accordance with the DMVA general safety rules. Immediately reports all workplace injuries or accidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to the supervisor. Attends all assigned QAPI training. Follows established policies and procedures in support of QAPI efforts. Supports QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings. Follows standard precautions. Uses required personal protective equipment such as gloves, masks, gowns, goggles, face shield, hearing protection, foot protection, etc. Performs other related work duties as required. **percentages are based on approximate time of day-to-day activities, not responsibility. |
