This position, in the Bureau of Health Promotion and Risk Reduction, Division of Nutrition and Physical Activity, in the Chronic Disease Prevention and Control Section, supports the organization’s mission to promote healthy lifestyles, prevent injury and disease, and to assure the safe delivery of quality health care for all Commonwealth citizens through its responsibility for assessing, planning, administering, implementing, and evaluating Diabetes Self-Management Education and Support initiatives on a statewide level as part of the Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes grant funded by the Centers for Disease Control and Prevention. This professional position is funded through a grant from the Centers for Disease Control and Prevention. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Coordinates all planning, implementation, and evaluation of goals and objectives established by the federal grant for diabetes self-management education and support strategies. Coordinates preparation and development of grant agreements, using Department of Health (DOH) procedures, for the delivery of outreach, education, and evidence-based interventions for the diabetes self-management education and support program to improve health outcomes for people with diabetes. Coordinates evaluation of public health programs, evidence-based interventions, and the improvement of health outcomes for people with diabetes using qualitative and quantitative data in collaboration with an evaluation partner. Develops and maintains expertise related to diabetes and the diabetes self-management education and support program including policy issues and resources. Creates and maintains partnerships to coordinate with other chronic disease prevention and health promotion programs to further program goals. This includes identifying and collaborating with delivery system partners, and partnership-building with community organizations, committees, task forces, and state and federal organizations. Secures funding for the DOH by seeking and applying for funding for diabetes control at the state and local levels as well as other public health funding. Contract Management Creates contracts, grants, work statements, program narratives, program budgets, budget projections, and spending plans as required by the Centers for Disease Control and Prevention (CDC) and DOH using and adhering to established guides for the utilization of federal funds. Develops funding announcements for local intervention programs, including scoring criteria and prescribed evaluation tools. Negotiates, administers and monitors grants and contracts for assigned diabetes self-management education and support strategies using status calls, monitoring reports, site visits, and other relevant monitoring and evaluation tools. Evaluates program compliance using funder and DOH guidance to ensure program goals and policies are being met. Monitors invoices and expenditures in accordance with state and federal guidelines to ensure funds are being expended appropriately. Communication Provides technical assistance to professionals throughout the Commonwealth and DOH using program expertise related to diabetes and diabetes self-management education and support. Prepares federal grant progress or annual reports and annual updates for the CDC within established time frames. Collaborates with and facilitates collaboration among national, state, and local organizations in the public, private, professional, and voluntary sectors to develop a vision and establish policies and programs to improve the health of people with diabetes. Attends national and state meetings of professional, voluntary, community, DOH office and other health organizations that address chronic disease. Prepares and disseminates materials such as fact sheets and articles for public health newsletters and other publications to educate the public. Prepares and delivers formal public presentations upon request using current information to ensure dissemination of information relative to diabetes self-management education and support and other evidence-based interventions. Develops outcome measures to improve program implementation and for compliance with federal grant funding using the Notice of Grant Award guidelines. Provides direction in the development and implementation of data collection, surveillance and intervention activities and ongoing evaluation and revision of program plans. Demonstrates an understanding of and ability to interpret and communicate relevant and available baseline statistics such as numbers and rates of deaths; incidence or prevalence; populations disproportionately affected; trends; and associated behavioral risk factors. Monitors trends and patterns to identify groups with or at higher-risk of disparities in diabetes-related health outcomes. Creates or provides data collection methods for analysis and program reporting. Conducts needs assessments on the health status of populations to determine program priorities and priority interventions for program and policy development. Recognizes and supports diverse perspectives in developing, implementing, and evaluating policies, programs and services that affect the health of people with diabetes. Solicits input from individuals and organizations with diverse perspectives throughout program development, implementation, and evaluation. Collects, analyzes, and interprets data from reliable and available sources for state and federal reporting and to make evidence-based decisions for public health program planning and monitoring. Analyzes facilitators and barriers to program success. Makes recommendations on changes in program plans to ensure they meet the needs of priority populations by using current data and ensuring changes are in accordance with existing federal, state, and departmental policies. Provides leadership to link resources of professional organizations, community health improvement partnerships and other state agencies to address disease management and modifiable risk factors. Works collaboratively with the Bureau of Health Statistics and Research, and chronic disease epidemiologists to direct, plan, implement and evaluate the DOH’s goals for increasing access to healthy foods, increasing physical activity, and changing chronic disease modifiable risk factors. Maintains compliance with federal and state policies and Departmental procedures. Demonstrate awareness of the vulnerable populations the organization serves by identifying, providing, and advocating for resources, services, communication methods, and policies that would help those populations achieve health equity. Travels throughout the Commonwealth and outside the Commonwealth as needed and upon request. Recommends revisions to legislation to enhance the utilization of public health program resources. Performs other duties as required/assigned. Public Health Emergency Response – This position may be given an assignment to support the public health emergency response mission of the DOH. Emergency assignments could at times be received with little or no advance notice and may continue for any period of time – ranging from a few hours to several weeks, depending upon the nature of the circumstance. The emergency assignments may at times be performed under adverse conditions and may require travel to another site other than the employee’s assigned headquarters for periods of time. Telework - This position will have the opportunity to work from home (telework). In order to telework, the employee must have a securely configured high-speed internet connection. If the employee is unable to telework, there is the option to report to the headquarters office in Harrisburg. |

This position reports to the Chronic Disease Prevention and Control Section Chief. Work is assigned both orally and in written form. Assignments are performed independently based on knowledge of standards and procedures. Supervisor reviews on-going work progress after completion or if there is a problem or exception. Work is reviewed for consistency with mission, goals, and performance objectives. Regularly scheduled meetings are held with employee to review progress toward meeting work objectives, and to convey updated information. Supervisor schedules additional meetings as necessary to discuss and review progress on specific work projects. This position independently assumes responsibility for program development, implementation and evaluation of assigned strategies and initiatives. Decision making occurs following interpretation and analysis of policies and procedures. Some decision making involves addressing new and unique issues and problems that may not have been addressed in the past. The employee is responsible to find information to develop solutions or approaches. Position provides technical or administrative services and coordinates activities with others outside its work unit. This position does not supervise, manage, or serve as a lead worker to other employees. |
