Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

Provides clinical and programmatic consultative and monitoring to community intellectual disabilities programs in the Northeast Region under the regulatory jurisdiction of the Department of Human Services to ensure the implementation of Department policy and compliance with State and Federal standards, legal requirements and established Departmental rules and regulations. The incumbent receives direct supervision from the Regional Program Representative Supervisor and receives technical oversight and professional direction from the Office of Developmental Programs Medical Director in Central Office.

Description of Duties:

Provides clinical consultation to county and provider program staff to improve the quality of health care to individuals in the Northeast Region.

Provides clinical consultation and interpretation to county program and provider agencies on regulations and policy relating to health care.

Assists in the development of regulations, bulletins, policies, and procedures related to health care.

Serves as principal liaison between the ODP Regional Office and ODP Central Office for all matters relating to health care.

Obtains and maintains ODP Certified Investigator certification. Conducts investigations as a Certified Investigator when requested.

Participates in on-site reviews of Northeast region facilities serving children with medical complexity.

INCIDENT/RISK MANAGEMENT
Review, process, and close all medical incidents entered into Enterprise Incident Management system (EIM).
Documents that would be reviewed may consist of discharge orders from physicians and/or hospital discharge orders, laboratory test results all that may require treatment or follow-up services on the part of the reporter. Processing consists of assuring that reporting entity has follow-up with treatment or follow-up services so that an incident can be closed. Assure that all related documents associated with the incidents that are The Fatal Five (aspiration, constipation, dehydration, seizure, and infection/sepsis) are included, if applicable.

DEATHS
Review all deaths at the initial review stage to determine if immediate follow-up is necessary and investigators are assigned where applicable; coordinate with al providers, AEs, SCOs to secure the required death documentation for individuals receiving waiver and base funding; investigate deaths on-site when necessary; review all medical records, lab results, medical consultation notes and hospital discharge orders that are related t the death prior to closing death incidents in EIM; complete the monthly mortality reviews in EIM.

OMNIBUS BUDGET RECONCILLIATION ACT (OBRA)
Review and prepare responses to OBRA assessments and extensions submitted to Docushare; maintain electronic OBRA records for Northeast Region; provide guidance and communication with AEs, AAAs, Field Operations, SCOs, other regional nurses, participant family members regarding the OBRA process; ensure that AEs and SCOs are transitioning individuals from nursing homes back to the community, if appropriate, in a timely manner; participate with state wide planning meetings and updating of OBRA process; communicate with OMHSAS and ORC state offices for dual targets.

HEALTH CARE QUALITY UNITS (HCQU)
Point person for Northeast regional HCQU related activities such as regional nurse’s meeting, SNU meetings, training initiatives, referrals for provider training as necessary; point person for monthly state-wide HCQU planning meetings; point person for PCHC reviews of Pennhurst class members in nursing homes.

OTHER DUTIES include and are not limited to:
Regional point person for Health Risk Screening Tool initiative
Duty Officer (DO) as assigned
Technical assistance for providers regarding medication administration
Technical assistance for providers regarding medical incident management and health care issues.
On-site monitoring/visits as needed/assigned

Decision Making:
Describe the types of decisions made by the incumbent of this position and the types of decisions referred to others. Example: In response to a customer inquiry, this work involves researching the status of an activity and preparing a formal response for the supervisor’s signature.

1.Through the view and analysis of data and records, make determination of compliance with program requirements and regulations and prepare report of findings for supervisory review.
2.As a result of monitoring activity, advise county and/or providers of any identified areas of noncompliance and recommendations for program change.
3.In response to written complaints and inquiries, research the situation/concern and draft response for supervisor review.
4.When new policies and procedures are developed or problems persist or recur, participate in the development of training material and provide training and technical assistance to ensure understanding and consistent implementation.
5.Based on the analysis of data and review of trends, inform supervisor of identified issues and consult with and inform appropriate program staff of issues that affect their areas of responsibility.
6.In preparation for onsite reviews and monitoring, identify sampling methodology and process for internal review.
7.When conducting on-site reviews and/or investigations, use any established tools and protocols and routinely keep supervisor advised of progress being made and any significant concerns identified.
8.In order to ensure that corrective action has been taken, and counties and/or providers have adhered to established deadlines, maintain tracking systems and advise supervisor when delays occur. Prepare letters to affected parties for supervisory review regarding key deadlines.
