Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. This position, in the Bureau of Community Health Systems (BCHS), supports the organization’s mission of providing infrastructure for the implementation of core public health functions in a high quality, effective and responsive manner that will positively impact the health status of Pennsylvanians. This licensed professional nursing administration position is responsible for managing, assessing, planning, implementing, and evaluating public health services and initiatives within the Northwest and Southwest Districts. This position supervises Community Health Nurse Supervisors (CHNSs) and Nurse Consultants and is the reviewing officer for all State Health Center staff. This position can be headquartered in any NWD counties (Clarion, Clearfield, Crawford, Elk, Forest, Jefferson, Lawrence, McKean, Mercer, Venango, Warren) or SWD counties (Armstrong, Beaver, Butler, Cambria, Fayette, Greene, Indiana, Somerset, Washington, Westmoreland). |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Reviews county level assessments with CHNSs annually to ensure that district and county health priorities are recognized and addressed. Plans and directs health promotion and risk reduction activities in the districts accordingly. Provides on-call 1-877 PA Health after-hours coverage for the Northwest, Southwest, and Southcentral Districts to ensure that urgent public health concerns are addressed in a timely manner. Plans and directs clinical services, emergency situations, special initiatives, and community activities according to BCHS Policies, Procedures, and Standing Orders to ensure the provision of effective and consistent public health services. Collaborates with the Bureau of Epidemiology and Bureau of Communicable Diseases to ensure that all reportable diseases, selected non-reportable diseases, and outbreaks are investigated in a timely, complete, and accurate manner to ensure patient and public health. Monitors community health programs by accessing public health information systems using data entry and disease surveillance skills to ensure improved health status for assigned programs/populations. Oversees the planning, directing, and implementing of district emergency preparedness plans to ensure the continuation of operations should an emergency occur. Provides consultation to DOH staff, supervisors, health professionals, and community partners on public health issues via phone, email, and in-person communication. Maintains a current knowledge of public health programs, issues, and emerging disease threats. Participates in continuing education programs to meet the requirements of the State Board of Nursing, including the Mandatory Child Abuse Reporting trainings. Coordinates annual skills competencies/trainings, assures continuing staff development opportunities are offered and encouraged, and assists CHNSs with orientation of all nurses in BCHS. Facilitates the development and sustainability of community partnerships in the Northwest and Southwest Districts, while integrating the goals and components of the State Health Improvement Plan into district and community activities. Participates in developing BCHS policies and procedures that are in compliance with Professional Practice Standards, Nurse Practice Act, and Department of Health policies and procedures to improve the health of the population. Ensures the provision of quality clinical services for Department of Health Programs including: TB, Immunization, HIV, STD, and Newborn Screening, through BCHS staff and DOH contracted providers. Collaborates with CHNSs to determine appropriate service provision and staffing to meet identified public health needs, ensuring that disparate populations and those experiencing barriers to care are identified and linked to available community services and resources. Represents the DOH on district/statewide committees and advocates for the development of public health initiatives. Ensures that job descriptions, essential functions, and performance standards reflect subordinates’ current job duties and responsibilities to assure that expectations are clear and outcomes are measurable. Participates in the hiring process by recruiting, interviewing, and recommending candidates for appointment, promotion, and reclassification to maintain effective staffing. Promote leadership development at all levels to empower the workforce. Facilitates student observations with colleges and universities to orient and engage students in public health. Participates in pre-disciplinary conferences and Labor Relations meetings when necessary. Consults with chain of command and Labor Relations when an employee’s actions/performance may lead to discipline. Conducts quarterly district-wide meetings, regularly scheduled supervisory meetings, and teleconferences as needed, providing updates and education to staff to assure a competent workforce. Demonstrate awareness of the vulnerable populations the organization serves by identifying, providing, and advocating for resources, services, communication methods, and policies that would help those populations achieve health equity. Performs the full range of supervisory responsibilities, including assigning, prioritizing, and reviewing work; maintaining position descriptions; conducting employee performance management; approving/disapproving leave and travel requests; participating in staffing activities; coordinating employee discipline; and training staff, including coaching and team building. Public Health Emergency Response – This position may be assigned to support the public health emergency response mission of the Department of Health. Emergency assignments could at times be received with little to no advance notice, and may continue for any period of time – ranging from a few hours to several weeks, depending on the nature of the circumstance. The emergency assignments may at times be performed under adverse conditions and may require travel to another site other than the employee’s assigned headquarters for periods of time. Position is permanently assigned direct supervision which includes completing performance evaluations, approving/disapproving leave, affecting discipline, and making hiring decisions for at least five direct reports. Supervisor’s input to decision making: Professional. Work is assigned by Bureau management for the geographical area and by particular needs within the area. The work is reviewed by the Bureau management during telephone conferences, management meetings, individual conferences, written reports, and periodic evaluations. Supervisor reviews on-going work progress after completion or if there is a problem or exception. Employee has authority to change internal procedures or approaches as needed within the accepted professional approach. Communication authority: Position advises outside its work unit. |
