Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The clinician shall provide clinical and consultative services in accordance with the treatment guidelines advocated by the National TB Coalition of America (NTCA), the American Thoracic Society (ATS), the Infectious Diseases Society of America (IDSA) and the U.S. Centers for Disease Control and Prevention (CDC). The clinician shall also adhere to all policies, procedures, and guidelines set forth in the Department’s TB Manual.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

• Provide clinical services which shall include the performance of tuberculosis examinations and other diagnostic and treatment duties relating to tuberculosis for patients having or suspected of having M. tuberculosis. Clinical services shall include, but are not limited to, taking patient histories, performing patient examinations, making diagnoses, prescribing tests and medications, and completing clinical records, including the documentation of diagnosis or suspected diagnosis and a plan of care.

• The clinician shall be responsible for medication orders, providing authorizations for chest x-ray and laboratory services, isolation orders, and other related activities.

• Orders tests and laboratory studies and reviews results to inform course of treatment and manage patients in accordance with program policies.

• Maintain current knowledge of various TB treatment regimens and appropriate drug dosages.

• Monitors patients for toxic effects of anti-tuberculosis medications.

• Determines when patients are no longer infectious, may leave isolation/home quarantine and are able to return to work/school.

• Provide clinical services at the specified clinic location (s), at clinic times mutually agreed upon by the Department.

• Provide consultative services to include the provision of a second opinion regarding patient care when the patient is already a client at a State Health Center. The consultation may occur in person or through electronic means (i.e., email, telephone). The request for consultation may be initiated by the State Health Center TB clinic physician, State Health Center Nurse, Public Health Physician, the TB Program Central Office, or a private physician.

• Provide consultative services associated with TB Cohort Reviews as requested. TB Cohort
Reviews include, but are not limited to, reviewing medical records as a systematic review of patients with TB disease and their contacts managed at a State Health Center within the State Health District to determine appropriateness of care provided. The TB Cohort review shall be conducted at the Department’s request.

• Shall participate in TB Clinicians’ Meetings as sponsored by the Department.

• Abide by the confidentiality provisions of the Disease Prevention and Control Law of 1955, 35 P.S. Section 521.1 et seq., specifically 35 P.S. 521.15, as if it were the Department. The Provider shall also comply with the Confidentiality of HIV-Related Information Act; 35 P.S. Section 7601 et seq. (Act 148).

• Comply with and not violate the Corporate Practice of Medicine Doctrine.

• Collaborate with Pennsylvania Department of Health staff, (including but not limited to Public Health Physicians, Community Health Nurses, Public Health Program Representatives and Public Health Advisors) and County/Municipal Health Department staff to have access to TB patients examined at Participating Provider sites to interview them as necessary.

Makes decisions related to the diagnosis and treatment of patients having or suspected of having M. tuberculosis. Consult with Department TB Program staff and medical consultants as needed.

Experience:

Staff providing services shall have appropriate and valid professional license to perform the required clinical services. Staff providing services shall be qualified to perform the required clinical services.
