Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

Responsible for the total nursing care and related services given to the residents on designated units in a Department of Military and Veterans Affairs veterans home ensuring adherence to HIPAA standards. Also responsible for establishing and maintaining a therapeutic environment with emphasis on rehabilitative restorative, skilled, supportive and Palliative Nursing care.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

25% Provide a standard of care within the ANA regulatory standards and professional code. Responsible for the nursing assessment, developing a written patient care plan, as well as coordinating the implementation of this plan with other nursing personnel. Evaluate residents’ response to nursing care and revise care plan to reflect residents’ condition and effectiveness of nursing care. Responsible for nursing health education and health counseling.

15% Administer medications as needed. Monitor to assure that it is done properly by assigned qualified personnel and maintain current medication records. Monitor completion of the Medication Administration Records and Treatment Administration Record.

15% Observe residents’ conditions; ensure changes are reported to appropriate staff. Record daily care performed for the residents on the appropriate forms and the approved electronic medical record. Use medical history and assessment tracking software such as Care Tracker and Ultracare.

10% Serve as charge nurse or in a leadership position. Teach subordinates new procedures and assist them in planning their work, upgrading their nursing care, and general performance. Evaluate the care given to residents by subordinates, keeping documentation on good as well as poor performance. Provide input to Registered Nurse Supervisor regarding subordinate work performance, as necessary. As a supervisor, perform duties such as planning, assigning and evaluating work, determining work priorities, interviewing and selecting employees, evaluating employee performance, approving or disapproving leave requests, initiating disciplinary actions, determining subordinate training needs, coaching and counseling employees, and resolving problems and disagreements.

5% Secure report of resident condition from off-going tour of duty, check residents. Assess this information and make daily assignments with other nursing personnel accordingly. May perform, direct, and supervise the nursing care given the residents on scheduled tour of duty. Give report to on-coming tour of duty. Receive reports from Licensed Practical Nurse regarding any issues that are significant.

5% Complete all admissions/ re-admission assessments and related documentation. Assist with admissions on any floor as needed.

5% Conduct environmental rounds on a daily basis, document findings, take corrective action and submit daily report to ADON/DON.

2% Communicate all concerns to the RN Supervisor on duty.

2% May make rounds with unit physician, and if needed, assist unit physician with examinations and/or treatments. Notify physician of residents’ conditions. Monitor transcription of physician’s orders and conduct audits on a monthly basis to assure that the orders are carried out.

2% Notify other departments of any needed services. Establish and maintain effective communication with resident, family and staff.

2% Support the facility QAPI efforts by attending all assigned QAPI training and following established policies and procedures in support of QAPI. Participate in PIP teams as assigned. Share ideas for improvement in the organization; support QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

2% Attend Care Conferences and team meetings, relaying team decisions to other nursing personnel.

2% Conduct monthly audits which will be presented at the Quality Assurance Meeting. Work with the Pharmacist to do the monthly medication exchange. Oversee the ordering of all medicine and supplies used. Oversee the periodic inventorying of all supplies and equipment and maintain appropriate records. Inspect nursing equipment and make recommendations for repairs, corrections, and improvements.

2% Prepare residents for transportation to Veterans Medical Center or other institutions for treatment.

2% May be responsible for two or more assigned units depending on shift. May be assigned to work on different units due to staffing emergencies or shortfalls.

2% Attend conferences, meetings, and seminars as well as all mandatory in-service training programs as part of ongoing education. Satisfactorily complete all required competencies.

1% Perform work in accordance with the DMVA general safety rules and safety rules for this position. Immediately report all workplace injuries or incidents to the supervisor; and/or immediately correct any safety hazards in the work area or report same to the supervisor.

1% Perform other related duties as required.
Follow universal precautions. Use required personal protective equipment such as gloves, masks, gowns, goggles, face shield, etc.
