Position Purpose:

Position responsible to consult and interact with individuals, facility and agency staff, family members and / or community agency staff and provide guidance and technical assistance for compliance with regulatory standards and meeting social service goals and objectives for individuals at Ebensburg Center.

Description of Duties:

1. Serves as the State Center Social Worker coordinator for IM4Q consumer satisfaction initiative. Responsibilities includes serves as liaison to statewide IM4Q committee and IM4Q advisory group, collecting / analyzing data. Survey outcomes are provided to the RSS's and unit treatment teams with goals and recommendations.

2. Participates in developing, implementing, providing and coordinating social service programs for assigned caseload.

3. Provides information in collaboration with the Interdisciplinary Team during pre-placement plans and discharge process. Reviews voluntary and involuntary individual’s intake, commitment, recommitment or discharge data and documentation Annually with the assigned county court and ID Team.

4. Ensures that the quality of services provided meets the needs of the individuals served and their families as well as Medical Assistance standards.

5. Coordinates annual on-site reviews of individuals served by the Case Managers from County MH/ID Program Offices and requests to obtain individual's service plan (Life Management Plan) from the BSU Case Manager by sending them monthly in advance ISP schedule and encourages their participation in program and placement plans.

6. Participates in the 406 commitment process by testifying at hearings for the individuals in their caseload. Court orders are received and distributed to Medical Records and individual's unit for file.

7. Interprets federal, state, departmental and facility social services legislation, regulations, policies and procedures; assists staff and administration communicating such information for understanding and application to ensure compliance for Department of Human Services and Department of Health. Must be familiar with Medicare regulations.

8. Develops methods, procedures and record keeping system for retrieval of Social Service data necessary for training, statistical information, research and computer process.

9. Serves as an advocate for individuals’ Human Rights and participates as committee member of Human Rights Committee.

10. Provides guidance and technical assistance to ensure compliance with the social service regulatory standards.

11. Participates in the development of each assigned individuals’ comprehensive individual care, treatment or support services plan. Ensures protective, supportive and habilitative / rehabilitative social services are provided to individuals assigned to caseload.

12. Ensures needed benefits and services are obtained in accordance with assigned individuals’ treatment plans and applicable mandates, regulations and policies.

13. Motivates and assists individuals to develop and make use of their potential for improving their social, economic, emotional, physical and mental health circumstances and situations.

14. Arranges for and encourages individuals to use appropriate public, private and community resources.

15. Serves on assigned statewide and facility initiatives as a lead worker representing Social Workers and to both provide input and bring back information to staff.

16. Familiar with county structure and system of MH-ID services.

17. Recommends changes to individuals’ social service treatment program as appropriate.

18. Responds to correspondence related to social services, court documents, parental or relative support, complaints or allegations of human rights violations, rate payments for care and maintenance, and public and private insurance reimbursements.

20. Member of the Quality of Life Committee. collects data, prepares reports and summarizes information to be presented at the Quality-of-Life meetings.
21. Participant in Colisanti Family Collaboration. Provides families with information in regard to Life Sharing opportunities and home and community-based services that are available to them.
22. Participates in planning of all Family Events, including family visits, Events and activities. maintains communication with families to encourage their participation in planned events.
23. Attends and coordinates with the Therapeutic Activities Staff the annual family picnic.
24. Travel as needed.

Serves as liaison between the facility Interdisciplinary Team, the county and community agencies to facilitate planning for the adjustment of individuals served at the Center and eventual return to the community. This is accomplished through the establishment of an on-going therapeutic relationship with the individuals and their families in receiving and sharing all relevant information concerning individual care and programming.

Assesses individual and family strengths and needs to formulate and maintain individualized social work and discharge plans by participating regularly as an Interdisciplinary Team member. This includes the completion of all pre-ISP information and documentation pertaining to the individuals' financial status, family information, base service unit involvement and placement prognosis as defined in a Life Management Plan / Community Placement Plan.

Initiates and/or answers correspondence and phone contacts to/from families, agencies and other interested parties to keep them informed of individuals' health, adjustment and program progress by being aware of individual's daily happenings, activities and services offered. This includes verifying the participation or non-participation of interested parties at the individual's annual review, processing medical billing communications, dental and medical permission forms, general health information, communications as specified in the facility's behavior support policy, unusual incident reports, leave of absence request, inter and intra institutional transfers, and the coordination of the completion of all forms as pertinent to the Medicaid Waiver programs, as well as notification of interested parties for special events as they involve the individual served.

Develops and implements individualized training program for individuals served to enhance self-sufficiency by improving social skills, mobility skills, community awareness and other related need areas.

Serves as liaison with substitute decision maker in the processing of consents.


Provides direct care services to individuals depending upon the individual's needs. These services include personal individual care as directed by the supervisor, letter writing and phone calling at individual's request, assisting individuals with shopping, delivering and reading mail to individuals as needed, logging packages and responding to sender on behalf of the individuals, completing placement referral applications, and providing pre-placement counseling to individuals, completing placement referral applications, and providing pre-placement counseling to individuals and their families, arranging and providing transportation for individuals to receive home visits, accompanying individuals on home visits, assisting base service unit interviews, court hearings, and workshop placements, as recommended by Interdisciplinary Team or supervisor. Provide follow-up services to the individual, provider, court, county and State Center as specified at the time of discharge.

Coordinates for individuals to obtain needed personal items as recommended by the Interdisciplinary Team through contacts with the family, guardian officer, or representative payee. This employee will monitor the funding available to each individual served in the assigned caseload in order to maintain an approximation of spend able funds per individual. He/she will serve as the liaison between the treatment team and the responsible payee of the individual served and will notify the RSS if funding approaches a level making the individual ineligible for Medical Assistance benefits.

Maintains a comprehensive social service record in the chart of the individual served for Interdisciplinary Team use in planning for the individual. This employee will prepare a complete individual case history for each assigned individual served and document all correspondence, phone contacts, and visits as relevant to the individual's total habilitation plan. An annual Social Work Assessment/Discharge Plan will be prepared prior to each individual's annual review and update of this plan will be recorded in the individual's chart at least every 90 days. Following the individual's stay at Ebensburg Center, this employee chairs the unit's discharge meeting, prepares the discharge summary and attends the admission staffing at the placement site.

Participates in unit Team meetings, Social Service meetings and in-services/out-services as scheduled to keep informed of current developments, regulations, procedures, etc., relevant to the on-going provision of services to the individuals served and their responsible parties.

Provides staff training in areas of normalization, rights of individuals served, Medicaid Waiver, and other related subjects to enhance the understanding of individuals' needs through the monthly planning and presentation of in-services.

Reviews Individuals Rights and Code of Conduct Annually

Performs other duties as assigned.
