Position Purpose:

This position is the Dual Diagnosis (DD) lead for the state centers, leads the Therapeutic Recovery Team (TRT) and is a liaison with OMHSAS on DD. This position reports directly to the Director of the Bureau of State Operated Facilities (BSOF) in ODP and supervises clinical and professional staff who support the people living in the state centers. As the Program lead for the TRT, the incumbent directs the highly specialized consultative, mobile team of staff who provide clinical assessment and support to individuals with Dual Diagnosis (ID/MH) in a variety of settings throughout the state. This position requires frequent overnight travel. The incumbent assures appropriate interaction with private providers, psychiatric hospitals, forensic units, county jails, state mental hospitals to support individuals at risk of admission to a state center. In serving some of the most difficult people in the human service system (ODP/OMHSAS/DHS), the TRT shows outcomes of successful transitions for individuals admitted to state centers and transitioning to community settings. Assure the completion of specialized diagnosis, technique and person specific trainings are developed and implemented in the state centers as well as with OMHSAS and community providers as requested. TRT has proven to be a valuable resource to the State Centers and ODP resulting in a reduction of employee injuries in the state centers and a reduction of restraints for the people living in the state centers. This position as the TRT Program lead and as a member of the ODP clinical team assists in diverting people from possible State Center admissions, the need for crisis involvement, inpatient psychiatric admissions, incarcerations, and restraints. Provides support to the State Center administration and Interdisciplinary teams serving people with a dual diagnosis in a continuum from diversion activities, admission preparations through discharge to a new community setting.

Description of Duties:

Duties and responsibilities include:
Develops, maintains and supports a training program for the state centers to improve the services and supports needed to support people with Dual Diagnosis and complex behavioral needs.
Coordinates training and information sharing to offer current therapeutic approaches to the staff in the state centers.
Facilitates meetings of stakeholders relevant to the provision of Dual Diagnosis (DD) services and supports, including OMHSAS, Administrative Entities, Supports Coordination Organizations, Health Care Quality Units (HCQU) and private providers so the appropriate services and supports can be developed and remain available in the community and facility-based settings.
This position requires frequent overnight travel throughout the Commonwealth. (multiple occurrences per month)
Develop and/or support the development of communications related to DD activities and providing training and presentations.
Provide DD support and coordination for the state centers and other entities as requested.
Present to stakeholder audiences throughout the Commonwealth through training, facilitating and directing teams for dual diagnosis; serving in the capacity of the public relations and project management role for dual diagnosis activities.
Develop and review programs with other ODP bureaus and commonwealth entitites both at the state and county level in order to expand the number of community-based supports for individuals who have a dual diagnosis.
Report out on activities related to DD support, training and consultative efforts on a monthly and quarterly basis.
Support and lead initiatives related to Dual Diagnosis and complex behavioral supports as assigned by the bureau director.
Support the QM management activities in the state centers and participate in focus group activities.
Work closely with the clinical leads in the Bureau of Community Services, Bureau of Supports for Autism and Special Populations as well with the clinical staff in other DHS agencies.

Lead and support the TRT staff in order to promote a healthy, safe environment with restraint free, person-centered, offering greater opportunity for increased independence, choice and control in a person’s life in the State Centers.
Provide oversight and direction to the team of professionals that provides assessment, crisis intervention, behavioral support planning and actions with active treatment to engage the individual.
Develop processes for the team to support stabilization, skill development, promote recovery and skill acquisition to support the person’s life trajectory in a consultative and training capacity.
Develop quality measures and positive behavioral support quality measures that are incorporated into the Support Plan of an individual and monitors the effectiveness of plan implementation.
Provide support to the State Center Interdisciplinary (ID) teams with plan development, training and data analysis while promoting best practices to keep individuals safe and restraint free in the State Centers.
Provide support to State Center ID teams serving people with a dual diagnosis in a continuum from diversion activities, admission preparations through discharge to a new community setting.
Develop and recommend training on best practices regarding DD, behavioral support, therapeutic techniques, assessments, data collection and analysis to State Center staff.
Provide leadership and direction to a highly specialized clinical team.
Ensure appropriate assessment and plan development for individuals at risk of admission to a state center.
Interact with private providers, psychiatric hospitals, state mental hospitals, forensic units, county jails, state centers, courts and families and travels frequently to locations throughout the state for meetings, to perform assessments.
Provide reports to BSOF Bureau Director, ODP’s Deputy Secretary, OMHSAS’ Deputy Secretary, DHS Secretary and staff on the activities of the TRT, assessments and recommendations for complex cases.
Facilitate the skill building of the staff who are part of the TRT team.
Evaluate the employee’s demonstrated job knowledge and essential skills , such as work policies, best practices, dual diagnosis, community resources and transition into the community setting.
Collaborate with the leadership of state centers and other state faciliities.
Participate in policy development for BSOF, ODP and joint ODP/OMHSAS policies.
Provide consistent follow up communication to TRT members and administration of the state centers, Bureau Director and Private ICF’s verbally and in written form about issues that require attention and/or action.
Engage in strategic planning for TRT and measurement in the achievement of the program.
Promote collaboration and effective communications within the bureau and partner, OMHSAS.
Actively look for new and innovative methods and techniques to improve the services provided to the individuals served by TRT.
Provide training to better support the Dual Diagnosis and complex case initiatives and direct supporters.
Supervise professional staff.
Directly supervise Residential Services Unit Managers involved in programmatically supporting individuals living in the State Centers with DD supports.
Significant travel throughout the state of Pennsylvania including overnights.
Performing other related duties.
