Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

Responsible for providing clerical support for Social and Rehabilitative Service departments under the Chief Social Rehabilitative Executive's (CSRE) supervision including: Psychology, Social Services, Adult Basic Education, Occupational Therapy, Therapeutic Recreation, Vocational Services, Volunteer Resources and Spiritual Services. This person is assigned to the CSRE’s office. These job tasks are carried out on the grounds of Danville State Hospital.

Description of Duties:

Work Effort Statements:
Provides clerical support for the CSRE. Services are also provided to the following S.R.S. Departments: Social Services, Therapeutic Recreation, Adult Basic Education, Occupational Therapy, Spiritual Services, Psychology, Volunteer Resources, and Vocational Services when required.

Adheres to DSH policies / procedures and maintains compliance with CMS / DOH / OMHSAS regulations.

CSP- Recording: Gathers CSP assessments, attends CSP initial and update meetings, documents the outcomes / decisions of the discussions occurring within these meetings, files, data enters, and maintains copies of all CSP related documents. Mails copies of the CSP document to those identified during the CSP discussion.

Responsible for completing, maintaining, and updating the Treatment Team Schedules through communication with Treatment Team Coordinators. Sends copies of the Treatment Team Schedules to the county to notify them of scheduled meetings.

Responsible for clerical duties for the CSRE and S.R.S. Departments when required.

Communicates directly with Social and Rehabilitative Services departments, supervisors, and staff. Prepares reports for the SRS Departments as requested.

Prepares storeroom requisitions and maintenance work orders for the S.R.S. office and CSRE.

Receives incoming telephone calls from outside of the department and hospital. Relays information to the appropriate staff.

In the absence of a supervisor or department director, accepts calls from S.R.S. department heads and staff reporting off duty. After accepting call offs, relays this information to the employee’s supervisor and unit/group areas. In the absence of the Clerk Assistant 3, e-mails the list of S.R.S. staff unavailable (off duty, training, etc.)for the day to SRS staff before 9 AM each morning.

Performs clerical duties for the Social and Rehabilitative Services Director (CSRE) and S.R.S. department heads by attending S.R.S. Leadership meeting and other assigned, meetings, taking handwritten notes and transcribing of meeting minutes, correspondence, reports, and types various forms from handwritten drafts. Digital recording of minutes for transcription may be utilized if meeting participants are in agreement.

Types inservice handouts and meeting agendas, correspondence, memos, schedules, policies and procedures, group descriptions, posters, invitations, mailing lists, time studies, assessments, invoices, performance improvement reports, key requests and any other typing/correspondence as necessary for SRS department heads in order to maintain accurate information/monthly statistics and up to date schedules.

Maintains and improves necessary job knowledge and skills by attending and participating in Education Department/Hospital groups, meetings, committees, conferences, and reviewing memos and directives in order to improve quality of services and self development. Completes required trainings in a timely manner.

Types and distributes consumer group descriptions when submitted by S.R.S. staff, assuring format and subject matter are correct, manuals on the units are current and that staff have access to updated shared files.

Maintains an inventory of office supplies and forms in order to keep adequate supplies available for CSRE and Social and Rehabilitative Services staff as needed.

When the CA3 is unavailable, requests copy machine maintenance when unable to correct the problem, orders paper, toner, staples.

Assists with set-up and participates in large group/ hospital-wide activities.

Compiles a packet of forms for each new admission to the hospital.

Assists with the paperwork for acquiring consumer identification documents as needed.

Updates the Patient Information Booklet and following a consumer admission, mails a copy to an identified family member or friend.

Provides clerical support to the Consumer Advisory Council.

Provides assistance and training as needed within the electronic health record.

Updates Holiday Active Treatment forms prior to major holidays.

Perform other related duties as required.

This employee is able to independently perform all duties listed above. Patient inquiries may be addressed independently if the answer to the question is known. When this employee requires guidance to complete a task, the inquiry should be directed to the CSRE.

Essential Functions

 8. Successful completion of CPR Certification training
 9. Ability to walk the length of the unit hallways numerous times per day
 10. Ability to physically lift and carry routine office supplies.
