The social worker is expected to fulfill duties using independent judgment along with general supervision. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Provide Direct Social Work Services to residents on assigned units. Duties include: Ensure the maintenance of an accurate psychosocial assessment for each assigned resident which contains information concerning family relationships, employment history, educational background, financial status, personal interests and availability of family/community support services. Ensure the integration of each assigned resident’s psychosocial assessment with their Total Plan of Care and Interdisciplinary Team Reviews. Maintain written documentation of the frequency and nature of social services provided to residents to meet regulations of accrediting and oversight agencies. Provide consultation to the Commandant, Medical Director, Clinical Services Manager, and other professional staff as needed. Counsel residents, their families, and others individually or in groups utilizing various treatment modalities such as supportive counseling, family therapy, crisis intervention, behavior modification, etc. to help them recognize issues and to explore methods for adjusting, modifying, and improving personal and social functioning and behavior. Represent the veterans’ home in community activities and programs related to health and welfare and/or community development. Serve as a member or chairperson of various veterans home committees as required. Manage discharge planning and referral to community agencies. Promote both departmental and interdepartmental cooperation. Promote productive relationships with outside agencies. Function as advocates for those residents who are dependent and/or helpless. Respond to complaints and suggestions where appropriate and communicate policy or response to issues in order to resolve concerns of residents. Attend all interdisciplinary meetings, case conferences, plans of care meetings, etc., when necessary. Assist family members upon resident death with funeral arrangements and grief counseling. As required, participate in the work of other social workers. Support the facility QAPI efforts by attending all assigned QAPI training; following established policies and procedures in support of QAPI efforts; participate in PIP teams as assigned; sharing ideas for improvement in the organization; and supporting QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings. Perform work in accordance with the DMVA general safety rules and safety rules for this position. Immediately reports all workplace injuries or incidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to supervisor. Perform other related duties as required. Use personal protective equipment when required such as gloves, masks, face shield, etc. |

Work is reviewed formally in six-month review and annual evaluations. Work is also reviewed in supervisory conferences on a weekly and as-needed basis. |
