Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Perform work as a Social Worker 2 for the Office of Vocational Rehabilitation with appropriate students with disabilities in local education agencies and identified schools in an assigned geographic area. Provide group instruction, information, referral and advocacy to students with disabilities to enhance their successful transition from school to services provided by the Office of Vocational Rehabilitation and eventual employment. Investigate student service needs by gathering and evaluating information available on the student’s current education levels, physical condition, age, disability severity, aptitudes, interests, abilities and other factors that will be relevant to their successful transition from school to employment. Provide information on the services available by the Office of Vocational Rehabilitation and other agencies providing services to individuals with disabilities. Identify available community and educational resources that provide services needed by the student, to include referral to appropriate agencies. Act as liaison to the staff of local education agencies and public and private agencies by: providing disability and vocational consultation, responding to telephone inquiries and letters, visiting and providing information on disabilities and the Office of Vocational Rehabilitation, providing in-service training on disability, conferring with agency staff to facilitate relationships to advance the vocational development of students with disabilities, and developing collaborative service provision to prepare students with disabilities to take full advantage of the services offered by the Office of Vocational Rehabilitation. Attend meetings with students, families, school and agency personnel to assist in preparing individualized educational and service plans that facilitate the successful transition from school to employment. Act as advocate on behalf of students with disabilities and their families who require assistance in obtaining services and comparable benefits by locating resources, informing students and families of their rights and making initial and follow-up contacts as necessary to obtain appropriate services that will facilitate the successful transition of the student from school to employment. Assist students with disabilities to develop positive attitudes towards work and help them aspire to becoming part of the workforce by utilizing interest inventories and other software to determine aptitudes and abilities that will lead to successful outcomes as a result of services provided by the Office of Vocational Rehabilitation. Promote public awareness about the services offered by the Office of Vocational Rehabilitation by providing trainings and presentations, attending conferences and meetings, and by participating in other events attended by personnel that have interaction with students with disabilities and their families. This includes interacting with and providing presentations to student and parent organizations. Coordinate disability-related services to meet the needs of students with disabilities to prepare them for services to be provided by the Office of Vocational Rehabilitation. Gather appropriate medical and educational information that will be utilized by Vocational Rehabilitation Counselors to determine eligibility for the state Vocational Rehabilitation Program. Prepare application packets for release to students interested in referral to the Office of Vocational Rehabilitation. Assist students in completing applications for services offered by the Office of Vocational Rehabilitation. Prepare completed application packages that include all relevant student personal, educational, vocational and medical information that will be required for Vocational Rehabilitation Counselors to take over the case and determine eligibility in a timely manner. This includes facilitating comparable benefits to obtain any necessary testing or diagnostic evaluations that may be needed to determine eligibility, such as advocating for current vocational and psychological testing being done by the school personnel. Utilize the web-based Commonwealth Workforce Development System (CWDS). Enter detailed record information directly into CWDS. Enter information into CWDS so that it can be viewed and shared. Generates correspondences within CWDS as needed. |
