Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. The Clinical Dietitian in this position is responsible for the planning, organization, and implementation of nutritional care for Individuals residing in a Department of Human Services facility for Individuals with developmental disabilities. Work is performed with a high degree of self-initiative and professional judgement. The Clinical Dietitian reports directly to the Director of Dietetic Services. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. The Clinical Dietitian maintains a level of competency acceptable to Selinsgrove Center and its Dietary Department. Participates in any Dietary Department performance improvement programs/initiatives according to program assignments and parameters, including diet accuracy, nutrition intervention outcomes, and menu evaluation analysis for nutritional adequacy. Completes on a timely basis, new admission and annual nutritional assessments, and Physician ordered nutritional consults for Individuals. Makes recommendations to the Physician regarding nutritional care and interventions. Collects information from charts, medical records, dietary records, and by talking with Individuals, medical staff, and living area staff/advocates. Thorough and accurate nutritional notes/charting is always maintained concerning Individuals’ nutritional care. Develops and documents policies and procedures related to nutritional care as requested. Maintains current diet records in living area charts, dietary files, and in the computerized nutritional management system. Documents all actions taken regarding patient care. Uses the computerized nutritional management system to ensure Individuals receive meals consistent with their prescribed diet and consistency by implementing menu changes for therapeutic interventions, substitutions for food preferences/dislikes, food modification, and prescribed calorie level. Completes data entry for Individuals participating in the selective menu program. Assists in menu planning for special events, holiday meals, and unestablished meal plans. Assesses needs for snacks, condiments, nourishments and other supplies for living areas and/or specific Individuals. Updates and posts standing snack, fruit, and vegetable delivery schedules. Establishes and implements training programs for general staff orientation and on the job training to direct care staff. Provides special instructions to food service staff and cooks regarding therapeutic diet preparation and service on the tray line as requested. Provides training to direct care staff, including creating training rosters, when nutritional care plans change that warrant immediate interventions. Attends and participates in scheduled Individual Support Plan (ISP) meetings. Attends and participates in Team meetings as requested. Serves as an active participant on the Dysphagia Committee. Provides and documents information regarding the nutritional component of dysphagia consults. Serves on other committees as requested. All communications are always to be professional. Adheres to all policies and procedures to ensure an efficient and continuous food service operation. Follows policy and procedures such as all safety rules and regulations in the Food Service Department and at Selinsgrove Center. During emergency and/or personnel shortages, alternate job duties may be assigned to ensure efficient and continuous Food Service Operations.
