Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The Registered Nurse Supervisor is responsible and accountable for planning, directing, coordinating, evaluating, supervising, and providing nursing services as necessary at Selinsgrove Center.

Description of Duties:

Is directly supervised by the Nurse Manager. Plans and organizes work under direction of Nurse Manager. Is responsible and accountable for the development, implementation, evaluation, and monitoring of treatment for assigned individuals or serves as an evening or night supervisor involved in planning, directing and evaluating the administration of Nursing Services. Directs non-professional staff in the provision of health and related services. Plans and organizes work to assure comprehensive nursing services are provided to persons residing in assigned area.

Is responsible for assuring all nursing services within assigned area comply with policies/procedures, rules/regulations/laws, and standards associated with Nursing Services (which include but are not limited to: Standards of Nursing Practice Licensure requirements for Pennsylvania Nursing; Intermediate Care Facilities for the Intellectually Disabled(ICF/ID) standards; Department of Human Services (DHS)/Office of Developmental Programs (ODP) directives associated with the Nursing Services discipline. Functions include monitoring, providing, directing, auditing and evaluating the delivery of nursing services and related health maintenance/restoration activities, risk management reviews from a nursing perspective and the facility’s Infection Control Program.

Participates in the development, review, training and implementation of Center policies and procedures.Functions include monitoring, providing and auditing/evaluating Facility Health Policies/Procedures, nursing interventions and related health maintenance/restoration activities, infection control, risk management, educating/training individuals and staff in health practices and related issues as well as achieving/maintaining compliance with facility and professional health care standards, guidelines/rules/regulations which include, but are not limited to, the Standards of Nursing Practice, Licensure requirements for Pennsylvania, Intermediate Care Facilities for the Intellectually Disabled (ICF/ID)standards associated with the Nursing Services discipline and to assure that optimal care and treatment (based on current professional practice)in a safe therapeutic environment is provided in assigned areas under the general supervision of the Nurse Manager. Directs non-professional staff in the provision of health and related services.

PHYSICAL AND MENTAL REQUIREMENTS OF THE JOB:

·Ability to provide emergency and life saving care to the people living at Selinsgrove Center.

PROFESSIONAL, TEAM AND GENERAL RESPONSIBILITIES OF THE JOB:

·Coordinates the delivery of Health Services for assigned people.
·Establishes contact with family members to discuss health related issues.
·Establishes and maintains communications with previous tours.
·Maintains a safe, therapeutic environment.
·Supervises nursing staff in identification and assessment of physical, psychosocial and/or mental health nursing care needs.
·Participates in the administration of direct nursing care and treatment as needed.
·Maintains continuity of all phases of nursing care.
·Evaluates the nursing process and nursing care and treatment services, recommends changes and provides follow-up.
·Investigates reported medication/treatment deviations, completes appropriate forms and recommends follow-up, changes in established procedures and/or discipline.
·Participates as a member of the IDT, serving as a resource to nursing staff and other members of the IDT.
·Participates on designated Center committees.
·Functions as QIDP in emergencies for behavioral interventions while adhering to Center practices and policies.
·Complies with DHS policies, state nursing laws and regulations and Department of Health Standards and corrects any noted deficiencies.
·Utilizes appropriate outservice training and participates in and/or provides center-wide training. Reviews current literature.
·Actively involves self in the community placement initiative to assure continuity of necessary clinical services.
·Represents self as a role model employee, advocating for individuals who have developmental disabilities and interacts appropriately with staff.
·Maintains standards of professional practice and legal and ethical practices in providing, monitoring, supervising and directing care.
·Assesses mental/health conditions and reports same to physician and Nurse Manager as needed.
·Documents Nursing Assessments, Physician’s Plan of Care changes and develops Nursing Plans of Care.
·Provides emergency and life sustaining care and evaluates the response to such care.
·Clearly expresses ideas and information, orally and in writing, in an appropriate manner.
·Identifies, initiates and evaluates training recommendations and appraises the effects of training provided.
·Exercises independent judgment in performing supervisor duties.
·Approves leave requests for subordinate employees.
·Maintains minimum staffing needs.
·Evaluates subordinates work performance.
·Receives and investigates grievances and complaints.
·Interviews and recommends selection of job applicants.
·Audits, monitors and evaluates the quality of nursing care provided.
·Assesses staffing patterns, assigns staff accordingly, identifies staffing needs and reports staffing deficiencies to the Nurse Manager.
·Communicates contractual and administrative rights and obligations to employees.
·Implements and participates in quality assurance/improvement/risk management and infection control activities.
·Provides input into development of policies and procedures.
·Prepares, reviews and responds to nursing care, treatment and services reports.
·Conducts monthly Nursing Department Meetings and Unit Nursing Meetings as assigned.
·Functions as the Supervisor of the Day as assigned.
·Conducts and/or assigns hospital visitation for people in community hospitals as assigned.
·Administers/conducts employee health related screenings as assigned.
·Performs related work as required.

PERFORMANCE STANDARDS FOR REGISTERED NURSE SUPERVISOR

1.0 JOB KNOWLEDGE
1.1 Functions within legal framework of professional nurse license.
1.2 Performs in accordance with DHS and facility policies and procedures.
1.3 Completes facility and departmental forms properly.
1.4 Implements medical regimens within prescribed written orders and ensures LPNs, direct care staff and RNs do the same.
1.5 Completes required and/or assigned training sessions to include annual retraining.
1.6 Understands and practices principles of positive approaches.
1.7 Possesses knowledge of current and best practices in the field of developmental disabilities.
1.8 Responds to emergency calls with appropriate first aid and treatment while adhering to guidelines established in the Health Emergency Response policy.
1.9 Plans, coordinates, and implements clinical training for new RNs and LPNs assigned to shift.
1.10 Possesses knowledge regarding availability, functionality, and use of emergency medical equipment and supplies.
1.11 Participates in continuing Education programs.

2.0 WORK RESULTS
2.1 Increases opportunities for people to have input and control into their overall health care.
2.2 Administers medications and treatments accurately and in accordance with the Center’s policies and procedures.
2.3 Performs nursing procedures with good technique and according to departmental policies and procedures.
2.4 Documents assessments and observations to include effects of pharmacological agents and therapeutic treatment.
2.5 Completes daily schedule as assigned that satisfies both coverage and bargaining unit requirements.
2.6 Submits written summary of the previous month’s activities involving yourself and those you supervise to the Nurse Manager by the fifth of each month.
2.7 Ensures that any identified deficiencies are corrected in a timely manner.
2.8 Participates in center wide committee or work groups when assigned.
2.9 Completes administrative duties as assigned by the Nurse Manager and as required by departmental process.
2.10 Completes performance evaluations and progress reviews on RNs based on standards you have established for performance.
2.11 Ensures the timely completion of routine duties.
2.12 Conducts on-going clinically relevant training to LPNs and RNs at your monthly meetings.
2.13 Conducts nursing rounds in all assigned areas to evaluate clinical problems.
2.14 Functions as QIDP when necessary.
2.15 Oversees use of medical restraints when in effect and ensures staff are properly trained on their use.
2.16 Conducts health emergency response readiness drills quarterly as assigned and completes necessary paperwork relevant to staff performance.
2.17 Prepares Death Summaries and attends Mortality Reviews as assigned.
2.18 Administers/conducts employee health related screenings as assigned.

3.0 COMMUNICATIONS
3.1 Accurately documents pertinent nursing observation in the record in a timely manner.
3.2 Reports to the physician pertinent nursing observations that require medical follow-up.
3.3 Establishes open communication lines with nurse supervisors on the other shifts regarding work needing completed and ensuring that the written and verbal shift reports contain all medical information which require follow-up or observation
3.4 Provides outside Medical Center/Hospitals necessary or requested information and receives the same from them.
3.5 Interprets physicians’ orders to direct care staff using verbal and/or written explanations.
3.6 Communicates self-training needs to Nurse Manager.
3.7 Offers professional input on the assignment and utilization of RNs and LPNs on your shift.
3.8 Initiates verbal or written feedback to the Nurse Manager on pertinent problems, needs, concerns and documentation of follow-up conducted for incidents involving the professional nursing skills of RNs and LPNs.
3.9 Conducts monthly meetings with RNs and LPNs to share information, solicit ideas, concerns and strengthen communication and documents meeting minutes.
3.10 Functions as an ID Team Member.

6.5 Ensures that RN and LPN performance standards are monitored on a routine basis, as required.
6.6 Contacts your immediate supervisor (or designee) to notify him/her of absence, in accordance with established time frames and, when necessary, provides documentation for use of sick leave upon return to work. (Reference DHS Manual Section 7173)

7.0 SUPERVISORY/MANAGEMENT
7.1 Ensures compliance to Nursing Care standards and regulations of reviewing bodies.
7.2 Ensures Performance Standards and Job Descriptions are current, communicated, and monitored for RNs and LPNs on your shift.
7.3 Provides direction to RNs and LPNs by setting priorities and establishing accountability for assignments.
7.4 Oversees quality of training offered by RNs to direct care staff.
7.5 Empowers RNs to implement strategies and principles of best practice.
7.6 Encourages RNs to offer resolutions to identified problems and encourage them to suggest changes that will positively effect the care given to individuals.
7.7 Requires RNs to share with each other information learned at training sessions outside the facility.
7.8 Ensures RNs and LPNs possess competence in the performance of nursing procedures.
7.9 Participates in supervisory training as scheduled and/or offered.
7.10 Monitors sick leave usage of RNs and LPNs to assure adherence to DHS’s Time and Attendance policy.
7.11 Performs 4 qualitative chart reviews monthly.
7.12 Interviews prospective employees as assigned and recommends employee selection.
7.13 Provides initial investigation of grievances from subordinates.
7.14 Conduct a minimum of one (1) extensive chart audit annually per subordinate submitting the appropriate documentation to the Nurse Manager.

General supervision by the Nurse Manager. Direction is received in the form of oral and written instruction and employee exercises independent judgment in performing supervisory nurse duties. This position provides direction to and supervision of Registered Nurse and LPN staff through conferences, oral and written instructions, daily rounds and periodic meetings. Performance evaluations are completed annually and/or as needed and performance is reviewed every six months.
