Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. Provides intensive social services to residents of SMRC. Is involved with preadmissions, admissions, case work, counseling, discharge planning, deaths and special projects. Work involves interacting with individuals, family members, guardians, agency staff and community agencies to plan, provide and coordinate needed social services. Work is performed in accordance with applicable rules, regulations, policies and procedures. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Planned working time percentages are approximate and can change from day to day based on operational needs. 40% Direct Resident Interaction • Assist Residents and Families during the admission process. Provide support and orientation to the facility. Assist residents in applying for benefits as necessary. • Interact with residents daily to assess and meet social service needs, with an emphasis on person centered care. • Advocate for resident needs and wants. • Provide basic supportive counseling for residents and families. • Assist resident and families for planning for end-of-life decision making. 25% Work with the Interdisciplinary Treatment Team • Attend and participate in Interdisciplinary Treatment Team meetings for assigned residents. Discusses assigned residents’ psychosocial assessments, family history, social service goals and interventions, and other relevant information in interdisciplinary team meetings. • Participate in the development and implementation of each assigned resident’s comprehensive individual care and treatment plan. Write Social Service goals and interventions. • Communicate regularly with interdisciplinary treatment team members in person, by phone, and by email. 25% Documentation • Write Social Service Assessments in the Electronic Medical Record, using professional standards for clinical documentation, and in accordance with established policies and procedures. • Write Social Service progress notes in order to document Social Work Interventions. 5% Discharge Planning • Assess Residents’ readiness for discharge from the facility. • Consult with the interdisciplinary treatment team, community agencies, and base service units, nursing home transition services to assist residents in discharge to community based or other settings. • Write discharge and transfer assessments to summarize a resident’s needs upon discharge from the facility. 5% Other • Participate in Department meetings and offers recommendations for improvement. • Serve on agency committees and facilitate Resident Council as assigned. • Perform related work as assigned. |
