The employee in this position is a Licensed Practical Nurse who provides nursing care on an assigned shift, in multiple living areas. Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Licensed Practical Nurse Responsibilities: 1. Utilizes the Nursing Process in order to maintain a high-level of wellness for each individual. This is done incompliance with Federal and State regulatory requirements and in-house policies and procedures. 2. Participates in the interdisciplinary process and the development, implementation, evaluation, and modification of the comprehensive Active Treatment Plan. This is accomplished by: A. Attending ISPs when on duty. B. Writing, implementing, evaluating and completing all the required documentation for medication programs in the assigned caseload, C. Maintaining a cooperative relationship with staff from all disciplines serving the unit’s population in order to maximize communication and ensure the most comprehensive and meaningful habilitation program for each individual. 3. Maintains open communication by relaying information and exchanging ideas in a constructive, courteous, and effective manner. This is accomplished by: A. Acting as a liaison to the physician, providing information and reporting observations, as well as assisting with examinations, as needed. B. Completing rounds with consultants as needed and participating in special health care interventions as indicated. C. Following procedures established for transcribing, implementing, and documenting the physician’s orders to ensure medical care is being provided as ordered. 4. Ensures effective and efficient nursing care delivery in all areas. This is accomplished by: A. Completing nursing functions, as described by the Nursing Standards of Practice, to meet the health care needs of the individuals at Ebensburg Center. Applying nursing techniques required for the intensive and continued care and treatment of the individuals. B. Performing related duties according to standards and target dates determined at the time of each assignment. C. Screening individuals daily for signs and symptoms indicating a need for change in the treatment plan and reports to the Registered Nurse in a timely manner. D. Completing observation and follow-up of any reported illnesses. E. Observing, interpreting, recording, and reporting the effects of pharmacological agents and therapeutic treatments. F. Completing monthly breast examinations on all female individuals, and male individuals as ordered by the physician, in assigned caseload. G. Completing the nursing responsibilities for the assigned caseload in accordance with Ebensburg Center’s policies and procedures. Completing documentation timely, legibly, and in accordance with the established policies and procedures. H. Familiarizing himself/ herself with all DHS and institutional regulations as applicable to nursing. I. Adhering to the current Ebensburg Center Infection Control policies and procedures J. Receiving verbal/written report at the beginning of the shift and giving verbal/written report to oncoming shift; assuring completion of the 24-Hour Nursing Report. K. Communicating any changes in health status to the RN and RNS. 5. Duties include, but are not limited to: A. Lifting, positioning, and transferring all assigned individuals with the assistance of other employees. B. Pushing wheelchairs and other adaptive devices. C. Performing emergency interventions such as CPR/Abdominal Thrust. D. Lifting, pulling, pushing, and carrying objects. E. Initiating safety mechanics (restrictive/therapeutic supports) as per Ebensburg Center Policy. F. Initiating and performing emergency evacuations. G. Understanding and interpreting verbal and written instructions. H. Coping with day-to-day problems and making reasonable decisions. I. Accepting reassignment to any area of the Ebensburg Center on an as needed basis. J. Accepting assignment of mandatory overtime when insufficient volunteers are available to meet the minimal staffing requirements. 6. Performs all related duties which are necessary in an emergency, in order to ensure the health and safety of those individuals who reside at the Ebensburg Center. 7. Maintains current Practical Nursing licensure and current CPR certification. 8. Maintains professionalism. This is accomplished by: A. Performing work responsibilities resourcefully and taking the initiative to provide for the health care needs of the individuals at the Ebensburg Center. B. Communicating effectively on a verbal and non-verbal basis with all staff, individuals, and families. C. Identifying strengths and weaknesses in overall performance striving to improve in areas of personal weakness. D. Enhancing his/ her own nursing growth and knowledge through active participation in meetings, conferences, in-service sessions, and out-service training programs. 9. Adheres to DHS time and attendance policy/ procedure. |
