Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. This is a professional social work position and the employee is a member of the Department of Social Work at Wernersville State Hospital. This position is responsible and accountable for providing a wide range of professional social work services and interventions to consumers who have a primary mental health diagnosis. This position provides services and interventions to each of the identified age specific subgroups of consumers, i.e., Early Adulthood (ages 18-36), Middle-Aged Adult (ages 37-64); and the Later Years (ages 65+). In addition to the specific tasks outlined below, this position must be cognizant of the ongoing aging process and the impact of passage from one life stage to another as well as issues related to cultural diversity. As a result, service delivery must be tailored accordingly to each consumer. Work is reviewed through conferences, reports and results achieved in meeting the social work program standards, goals and objectives. This employee is also the lead worker whose duties focus on the Continuity of Care. This employee maintains databases and assists consumers with a dual intellectual Disability Diagnosis and those involved with the Criminal Justice System. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. • Ensures services are relevant and appropriate to the individual consumers’ age group and cultural background. • Conveys information regarding the efficiency of operations, social worker outputs and any problems in the social work service delivery system. • Identifies and discusses options for alleviating problems or improving the social work service delivery system with the Social Work Supervisor and assists the Supervisor in implementing changes in current measures. • Interprets Federal, State, Department of Human Services and Department of Social Work regulations. • Ensures compliance with the Medicare and Medical Assistance criteria related to timeliness and quality of documentation. • Links consumers with systems that provide them with resources and opportunities by providing information and referring to resources which are relevant and specific to the age of the consumer/life stage/cultural background. • Assists consumers in applying for medical insurance and Social Security benefits. • Assists consumers in obtaining state identification, Social Security cards and birth certificates. • Establishes communication links among the hospital, consumer, family and supports, county liaisons and community providers in order to promote and facilitate continuity of care. • Complete sections of the court commitment petitions and review consumer rights regarding their court commitments and commitment hearings. • Participates in the Family Education and Support Program as assigned. • Serves as an advocate for individuals’ human rights. Functions as a core member of a treatment team by: • Completes admissions for assigned unit and as designated by the Social Work Supervisor or Social Work Manager. • Completes psychosocial assessments by interviewing the family, assessing the consumer and obtaining collateral information. • Notifies consumers of their rights during the admission process, throughout the course of their treatment and in preparation for discharge. • Facilitate court commitment hearings by bringing consumer to the hearing as appropriate and testifying as needed. • Reviews patient records as assigned, prior to the team meeting, to assure all relevant information is available from the disciplines involved in the plan. • Prepares necessary assignments, transfers, referrals and discharge documentations to ensure the least restrictive environment for meeting individuals’ needs. • Attends all treatment team meetings, commitment hearings, Community Support Plan Meetings, and court hearings as assigned. • Invites family members and county liaisons to treatment team meetings. • Contributes necessary information to the treatment planning process. • Implements treatment plans and assuring compliance. • Provides case management, implements treatment plans and monitoring consumer progress towards treatment goals. • Provides direct patient care as assigned by the treatment team leader. • Participates in individual and/or group programs provided to patients using an integrated multidisciplinary approach. • Provides feedback of progress to the patient, family, county liaisons and team members. • Communicates all relevant and significant information to the Treatment Team verbally and in writing via progress notes consistent with the Department, Medicare and Medical Assistance requirements regarding interventions, in order to provide consistency of care. • Maintains and improves necessary job skills by participating in staff meetings, training programs, seminars, and regularly scheduled supervisory conferences to improve the quality of consumer care. • Acts as a role model demonstrating professional behavior and attitudes to inspire staff to meet professional standards and to encourage persistence of effort in resolving the difficult problems presented by consumers. • Provides social work coverage that includes: Treatment Team Meetings, facilitating groups, accompany consumers on trips, court hearings, completing assignments & tasks, attending meetings and other duties and responsibilities as assigned. • Develops & implements social service discharge/case closing/ transfer and referral plans. • Reviews consumers’ chart to assure compliance according to regulation, best practice, policy and standard. • Ensures that needed benefits and services are obtained in accordance with assigned individuals’ treatment plans and applicable mandates, regulations and policies. • Works closely with the consumer, families, county liaisons and community providers to develop and implement a comprehensive discharge plan to support the consumer through the discharge process and successful reintegration into the community. • Other related Social Service duties as assigned. SW2 is the lead in support of the continuity of care for consumers by: -Completes US Justice Portal reports. -Maintains databases regarding Criminal Justice involvement such as 304gii and SORNA. -Maintains databases regarding competency hearings. -Provides consultation to other social workers regarding SORNA and 304gii process. -Informs treatment team of all documentation related to criminal justice involvement at the time of admission. -Ensures consumers are placed on the Duty to Notify list. -Maintains IDD database. -Provides consultation with other social workers regarding the process to obtain the necessary documentation to substantiate an Intellectual Disability Diagnosis. -Discusses potential conflicts and complex case management details related to Intellectual Disability Services and Criminal Justice needs with the Social Work Supervisor and the Social Work Manager. -Completes additional tasks related to Intellectual Disability Services and Criminal Justice needs as assigned. |
