Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This is a professional position responsible for providing counseling and case management services, through the application of casework, group, or individual counseling methods and techniques, to reentrants housed in a Community Corrections Center (CCC).

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

(40%)- Assists reentrants with setting and achieving goals related to employment, training, education, housing, and community service and maintains/updates the Reentry Focus Plan (RFP) from reception through release. Provides crisis intervention, conducts assessments and makes necessary referrals. Assists reentrants with home plan development and submission. Equips reentrants to take on the responsibility of living at their home plan. Serves as a staff-advocate for reentrants during the process of obtaining identification, medical assistance, social security, etc. and accompanies reentrants who require enhanced support, when necessary.

(40%)- Conducts a thorough orientation with reentrants to include a case file review and one-on-one discussion. Conducts reentry focused workshops, groups, and seminars that allow reentrants to practice pro-social behavior. Guides reentrants toward family reunification and the development of pro-social relationships and leads on-site support groups utilizing Reentry Peer Guides as facilitators. Applies cognitive based interventions, including Brief Intervention Tools (BITS), Motivational Interviewing (MI), Effective Practices in Community Supervision (EPICS), Carey Guides, etc.; Documents reentrants’ progress and maintains an Electronic Case File for each active reentrant. Complies with Prison Rape Elimination Act (PREA) standards related to position and utilizes the PREA Risk Assessment Tool as directed in policy.

(15%)- Reviews referrals and participates in discussions (videoconference, teleconference, in-person) with State Correctional Facility, Parole, and Bureau of Community Corrections staff prior to reentrants’ reception at the CCC. Conducts outreach and maintains current knowledge of social services agencies to ensure reentrants are successfully connected to community resources. Represents the CCC on community advisory boards and reentry coalitions and coordinates community service projects and volunteer opportunities for reentrants. Participates in hearings, court proceedings, and meetings. Participates in the American Correctional Association (ACA) accreditation process.

12. Hear and identify sounds (voices, whistle, buzzer, lock release, scuffling, sudden quiet)

Decision Making:

The incumbent develops a detailed and individualized RFP for each reentrant that addresses programming, treatment, employment, housing, and family reunification needs; reviews RFPs at select intervals to assess the reentrants progress and makes necessary revisions; and participates in infraction/violation hearings.
