Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This position serves as a Restorative Nurse Aide and is responsible for maintaining optimal physical, mental and psychosocial care by providing on-going rehabilitative and therapeutic treatments adjunct to initial therapies.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

Initiate an individual restorative program as designed and established by Physical, Occupational and Speech Therapy, and the inter-disciplinary team for each resident; collaborates with the restorative program leader, therapy staff, charge nurse, and nursing supervisor.

Observe and record signs/symptoms indicating the resident's response to the treatment program, including the resident’s improvement, specific goals met, change in condition, or noncompliance with program.

Maintain updated records of resident’s attendance and progress or non-participation for each session on Monette Progress Notes/Restorative Nursing flow sheet.

Observe residents’ conditions; ensure changes are reported to appropriate staff. Record daily care performed for the residents on the appropriate forms and the approved electronic medical record.

Utilize prevention measures to ensure quality of life and to maintain functional levels; provides opportunities for residents to maximize functional independence and prevent loss of existing capacity for activities of daily living (ADL) such as, feeding self, combing hair, washing hands and face, dressing, bowel and bladder training.

Guide residents in the use of assistive devices such as walkers, canes, crutches, wheelchairs, and adaptive equipment. Recognize the resident’s needs such as range of motion, ambulation, and limitations.

Transport and escort residents to various therapy appointments. Assist residents in the use of apparatuses in physical therapy such as the arm bike, parallel bars, exercise weights as well as stretching, upper body, and range of motion exercises.

Teach residents to make beds, change linen, care for bedside units and personal possessions within the resident’s limitations.

Assist at mealtime and teaches residents proper eating habits, reasons for therapeutic diets, and the nutritional value of foods.

Provide input and make recommendations for resident’s discharge from the program to the restorative program leader and Physical, Occupational and Speech Therapy staff.

Participate in inter-disciplinary team meetings, conferences and in-service education programs.

Monitor ambulatory devices and other therapeutic equipment for cleanliness, functioning and breakage. Notify supervisory staff of any concerns noted in these areas.

Assist with the admission process for new residents in order to provide guidance and support in a new situation and location by introducing residents to roommates, other residents, and staff.

Develop care plans for each problem and establish measurable goals and interventions specific to each individual resident. Periodic assessments and evaluations of each problem must be documented by a licensed nurse.

Demonstrate restorative care techniques to floor CNA’s for appropriate Level II maintenance of function.

Perform direct resident care within the scope of nursing care plans and individual restorative programs. This includes, but is not limited to, assisting residents with ADL’s, providing comfort measures, aiding in toileting, performing resident hygiene and nutrition measures and assisting with maintaining, retraining feeding patterns.

Perform resident safety measures of applying safety devices, splints, braces, anti-embolism stockings and elastic bandages.

Accompany residents to on or off ground appointments or activities as needed in order to provide continuing medical care and encourage involvement of the residents in social activities.

Help with housekeeping tasks, such as making beds, changing and taking inventory of linens, sterilizing and cleaning equipment and counters, to maintain a neat and clean environment.

Perform other related duties as required; to include other duties and responsibilities that fall within the scope of a Nurse Aide.

Work is performed under the direct supervision of the restorative program leader, therapeutic (physical, occupational and speech therapy) staff, charge nurse and nursing supervisor. Resolve routine problems within the scope of education and training.

Essential Functions

 1. Use various medical equipment, and transports and escorts residents on and off the premises
 2. Lift/move/re-position residents with assistance as needed
 3. Gather data, collect and record vital signs and specimens, report changes in resident behavior
 4. Serve and collect trays, assist in feeding
 5. Perform Activities of Daily Living and participate in rehabilitative programs for residents
 6. Wear protective equipment such as mask, gloves, and goggles
 7. Respond to alarms, calls for assistance and emergency situations
 8. Stand, stoop, push, pull, reach, lift, bend, kneel, twist, walk and grasp
 9. Communicate effectively verbally and in writing
 10. Apply ointments, lotions, and dressings to residents
