Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

This position prepares and maintains medical records for the Department of Corrections (DOC) State Correctional Institution (SCI) at Phoenix.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

30% Prepares and maintains electronic, paper, and film medical records. Uploads, merges, and extracts information in the Electronic Health Record system. Updates Medical Assistance Identification (MAID) numbers. Reviews and cross-references medical records for completeness and accuracy. Ensures that medical reports and documented entries are in accordance with documentation practices; DOC policies; and federal, state, or accreditation standards applicable to health care. Refers inadequate or incomplete records to health care staff for corrections. Provides documentation to the Corrections Health Care Administrator for patterns of inadequate or incomplete health records and the staff responsible.

30% Files, withdraws, and issues health care records in accordance with all applicable policies, regulations, and guidelines on confidentiality as well as the legal statutes. Completes Intra-System Medical Records Transfer Checklists. Delivers and retrieves records as needed. Composes and types routine requests for medical information from other facilities, hospitals or agencies. Prepares and transfers records for inmates paroling or being transferred from the institution. Notifies Health Care staff of the discharge status of inmate to other facilities and/or parole.

25% Archives hardcopy and electronic files for inmates that have left the system. Ensures that all paper records are provided to the State Records Center in an accurate and timely manner. Ensures that all inactive film and x-ray records are filed appropriately.

5% Updates and maintains tracking logs for monitoring release of information, DOC medical staff requests for information, requests from other facilities for medical records, etc.

5% Prepares periodic statistical reports on such subjects as morbidity, mortality, utilization of services and the statistics. Verifies medical coding included on various reports as needed. Prepares records for management reviews.

5% Other related duties as assigned.

Detects and resolves regular and/or routine problems and issues. Independently works with health care staff to ensure that medical records are accurate. Resolution of any irregularities or issues not outlined in clear policy and procedures require supervisory approval and/or guidance.
