Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. The employee occupying this position is a Pennsylvania Licensed Registered Pharmacist responsible for providing high quality pharmaceutical care and products for the consumers of Wernersville State Hospital (WeSH). The pharmacist's primary responsibility is the filling of the correct drug and strength for the correct consumer, with the clearest directions possible, with the least possibility of adverse reactions. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Fills medication orders and prescriptions and labels containers in a manner described in the Pharmacy Procedure Manual for hospital use. Checks all orders for adverse reactions, duplications, incompatibilities and allergies, using the consumer profile in the computer. Applies auxiliary labels where appropriate. Packages medications and cleans drug containers when necessary. Reviews consumer files for completeness and accuracy of orders using accreditation agency guidelines. Conducts retrospective drug reviews when requested or necessary. Consults medical staff regarding any concerns of a consumer's drug regimen and documents same. Checks drugs received in the Pharmacy against what was ordered and insures that received drugs are in acceptable condition with proper dating and quantity. Inspects expiration dates of pharmacy inventory and maintains inventory and associated paperwork to assure accountability. Performs routine inspections and maintains pharmacy-related inventory of storage areas throughout the hospital, including med-rooms, crash carts, clinic locations and the after-hours medication supply. Places the expiration date on the prescription label if the medication is not used much or expires in the near future. Manages outdated and recalled drugs. Provides information on use, dosage, side effects and safety of medications to patients, Nursing and Medical staff. Gathers and assesses drug information from available sources and makes information available to hospital staff when necessary. Assists in the maintenance of departmental-related records, according to DHS regulations, WeSH policies, State and Federal law and accrediting agencies. Reports possible adverse drug reactions, dispensing errors, and drug distribution system variances. Assists in the acquisition of QI information and records for the departmental QI program. Fills and enters pharmacy-related orders into the consumer’s computer file using the QS/1 computer program. Prints the physician's monthly rewrites and Medication Administration Records as necessary. Attends meetings or serves on committees, as determined to be necessary by the Chief Pharmacist or Administration. Obtains and maintains CPR Certification and utilize skills as necessary. |

Example: In response to a customer inquiry, this work involves researching the status of an activity and preparing a formal response for the supervisor’s signature. The pharmacist shall be responsible, without prior approval: to prioritize what issues and jobs need to be addressed or completed; to assign the pharmacy technician functions that are listed on their job description(s); to make decisions concerning physician's orders, (i.e., are all the necessary elements included in the order?), and to decide whether the attending physician needs to be contacted because of a potential drug allergy, interaction or duplication. The pharmacist shall report any problems with current polices/procedures to the Chief Pharmacist and, also, report any significant variances or errors to the Chief Pharmacist, ASAP. |
