The Licensed Physical Therapist provides professional services for adults with mental and physical illnesses from all programs on campus. The physical therapist is responsible for evaluating individuals in order to determine the cause or nature of the condition, situation or problem; the prognosis; appropriate interventions/plan; and adjustment to the treatment plan as required. Goals of all treatments should be to maximize patient independence, safety, quality of life and should be provided in an atmosphere of dignity and trust while maximizing discharge potential and inclusion in their community. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 1. Treats patients in all programs and settings on campus. 2. Conducts physical therapy evaluations with a physician’s order and administers medically prescribed physical therapy interventions to patients, in group or individualized settings, by the application of a wide variety of accepted treatment modalities of therapeutic and rehabilitative value. These modalities include, but are not limited to; therapeutic exercise, gait and transfer training, heat & cold, manual therapy, massage, ultrasound, TENS, electrical stimulation, positioning, splinting/bracing, staff and patient education. Any treatment should be 1) individualized and patient-centered; 2) provided with the goal of maximizing patient independence, safety, quality of life and inclusion in their community 3) provided in an atmosphere of hopefulness, dignity, trust and free from stigma; 4) integrated into an overall plan to address mental health, physical health, substance abuse disorders and overall wellness. 3. Screens individuals in order to provide baseline information regarding functional mobility, gross motor skills, pain, balance and fall risk and recommends PT evaluation as appropriate. 4. The physical therapist has responsibilities to the treatment team and participates in a group effort to increase the health, safety, independence and quality of life of patients at Torrance State Hospital by attending treatment team meetings, participating in discharge planning and maintaining open and timely communication with all participants. 5. Department and systems-wide, the licensed physical therapist is focused on customer service and quality improvement of provided services. Completes risk management work as needed. 6. Arranges and coordinates the scheduling of individuals for physical therapy program services and treatments in cooperation with other disciplines. 7. Accompanies patients on outings on and off campus. 8. Attends and participates in health and safety training programs like CPR, Mandt Advanced Training, Fire Safety and Emergency Response and Evacuation, and utilizes training as necessary. 9. Demonstrates and teaches staff in methods of maximizing the patients’ comfort, gait and transfer skills, skin integrity and strength and flexibility, while minimizing risks to patients and staff. 10. Maintains professional standards and professional skill development to assure up-to-date, quality physical therapy service delivery to all individuals. 11. Serves on Torrance State Hospital committees in order to provide professional expertise and personal insight. 12. Maintains stock/orders supplies, equipment and resources for individuals and the PT department in order to 1) Maintain and/or improve provided services; 2) Increase the safety of patients and staff, and 3) Improve the quality of life for persons residing at the Hospital. 13. Establishes open and honest interpersonal relationships and communication with supervisor, staff, students, volunteers and community providers. 14. Adheres to established standard for patient confidentiality. 15. Completes accurate, concise and timely clinical records including evaluations with plans of care, daily progress notes, group notes and screens. 16. Attends departmental, supervisory, treatment team and other meetings as assigned. 17. Able to complete all additional requests, forms and scheduling to ensure groups, treatments and departmental activities can be completed as scheduled. 18. Participates as a member of a team in conducting/assisting in special building events, health fairs and grounds-wide events as directed and scheduled. 19. Maintains a safe and clean working environment. 20. Displays a constructive use of time, takes initiative and works effectively with little oversight needed. 21. Reports to the Chief Physical Therapist. 22. Responsible for utilization of Employee Self Service (ESS) through MyWorkplace. Creates and submits leave requests, and travel requests; participates in online ESS training, reviews and updates personal, payroll, and benefits information. 23. Observes changes in mental or physical status, behavior or attitudes of patients and adjusts programs/goals as necessary. Communicates the changes to the appropriate staff. 24. Assists in program and policy development and performs related duties as required/assigned to meet the needs of patients, department and institution. Employee is expected to abide by Hospital and Department of Human Services rules, regulations and guidelines in the performance of their job. |
