Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. The employee in this position is responsible for coordinating and overseeing the full collaborative, interdisciplinary assessment, and care planning process for skilled care residents at the veterans’ home. This process includes comprehensive resident assessments; care coordination and planning; resident advocacy and teaching; facilitation of open communication among care team members, the resident and family; collection and transmission of data for the purposes of quality improvement; and adherence to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements when applicable. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 25% RAI Development / Regulatory Compliance: Serves as a Registered Nurse Assessment Coordinator (RNAC) responsible for the coordination of the processes related to assessment of residents in the assigned SVH. Responsible for the development, implementation, and evaluation of the assessment and care planning process accordance with applicable Centers for Medicare and Medicaid Services (CMS), Veterans Administration (VA), PA Dept of Human Services (DHS) to promote the resident’s quality of care and life in a State Veterans Home (SVH) nursing facility, including individuality, safety, wellness, satisfaction, and dignity. Complies with all federal and state regulations regarding completion and coordination of the RAI process. Reports any difficulties obtaining information from the Interdisciplinary Team Members for completion of the MDS/RAI process to the Nurse Manager 1 (assistant director of nursing-ADON). 25% Resident Assessment: Assists with coordination and management at the morning and afternoon clinical report meetings to include review of resident care and the setting of the Assessment Reference Date. Monitors MDS and care plan documentation for all residents. Ensures documentation is present in the electronic medical records to support accurate MDS coding. Maintains current MDS status of assigned residents according to state and federal guidelines. Supports the time frames and due dates of MDS schedules. Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. Complete, review and assist the Interdisciplinary Team Members with the accurate completion of Federal, CMS, VA, State, BVH and SVH required observations, forms and documentation. Ensures all required sections of the MDS are completed by the Interdisciplinary Team Members prior to submission to appropriate regulatory agencies. Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, residents, and family members. Reassesses and interviews residents as prescribed by federal and state regulations, including annually, quarterly, with any significant change in status, and as needed. Completes electronic submission of required documentation and all appropriately assigned clinical reimbursement paperwork in a timely manner. Periodically travels to various locations to conduct level of care assessments on admissions and readmissions to the SVH. 25% Person-Center Care Plans: Responsible for the initiation of the baseline care plan and development of the comprehensive nursing Plan of Care ensuring it is person centered and in accordance with applicable CMS, VA, DHS, BVH & SVH regulations and guidelines. Documents any significant change/reassessment in the residents’ plan of care and ensure that the appropriate interdisciplinary team members (IDT) are notified. Attend interdisciplinary team meetings and other meetings to gather information, communicate changes, and maintain and update records. 25% Quality Assurance and Process Improvement Responsibilities: Participates in clinical care rounds with other healthcare professionals when requested. Leads teams, manages projects, prepares reports, and delivers presentations and information in SVH assigned meetings such as, but not limited to, Clinical Report, Quality Assurance, Behavioral Health, Gradual Dose Reduction, Restraint Reduction and Plan of Care meetings. Communicates with SVH leadership regarding compliance related to the MDS, Care Plan process and medical record documentation to ensure quality of job responsibilities and resident needs are met. Participates in incident reporting, investigating and root cause analysis to support resident care and safety needs. Other standards: Maintain positive, constructive working relationships with co-workers; remain aware of the needs and sensitivities of others. Conduct job duties in compliance with established BVH and SVH policies, guidelines, and procedures. Maintain and improve job skills and competencies by participating in and attending staff training programs in order to improve the quality of resident care. Maintains and advances knowledge and skills in the area of nursing and the RAI/MDS process. Obtains and maintains Cardiopulmonary Resuscitation (CPR) certification. Performs work in accordance with the DMVA general safety rules. Immediately reports all workplace injuries or accidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to the supervisor. Attends all assigned QAPI training. Follows established policies and procedures in support of QAPI efforts. Supports QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings. Follows standard precautions and enhanced barrier precautions. Uses required PPE such as gloves, masks, gowns, goggles, face shield, hearing protection, foot protection, etc. Performs other related work duties as required. Obtains a Registered Nurse Assessment Coordinator Certification (RAC-CT) within twelve months of hire and maintains a RAC-CT certification from the American Association of Post-Acute Nursing (AAPACN). **percentages are based on approximate time of day-to-day activities, not responsibility. |
