The Social Worker 1 is responsible for providing a range of professional social work services, support, and interventions for residents of the facility. Work involves interacting with residents, residents’ representatives, and clients to provide and coordinate needed social services. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. 30% Participates as a member of the interdisciplinary care plan team by completing psychosocial assessments on admission, annually, and quarterly; and implements interventions that will assist and support the residents' individual needs and preferences, customary routines, concerns, and choices. Attends and participates in the IDT meeting for assigned residents and maintains accurate and complete written documentation. Ensures all charted documentation is informative of the frequency and nature of social services provided to residents to meet BVH and SVH policy requirements and regulations of accredited agencies. 25% Completes assigned sections of the resident assessment instrument minimum data set (MDS) on admission, quarterly, annually, and as needed basis. Completes behavioral assessments and implements appropriate person- centered interventions for residents, integrates each resident’s psychosocial assessment with their person-centered plan of care, as part of the health care team planning process. 20% Provides direct social work practice for residents with behavioral and psychosocial requirements and needs. Provides counseling or uses behavioral management techniques to help the resident with emotional issues or behavioral problems. 15% Counsels residents and family members in various treatment modalities, such as supportive counseling, problem solving, education techniques, and social skills training to help resident and/or family members to recognize the problem(s) and to explore methods for adjusting, modifying, and improving personal and social functioning and/or behavior. 10% Assesses residents’ readiness for discharge from the facility and works with the resident, family member(s) community liaisons, IDT, to implement a comprehensive discharge plan to support the resident thru the discharge process. Assists with coordination of transportation, and summarizes residents needs upon discharge from the facility. Maintains positive, constructive working relationships with co-workers; remain aware of the needs and sensitivities of others. Conducts job duties in compliance with established BVH and SVH policies, guidelines, and procedures. Maintains and improves job skills and competencies by participating in and attending staff training programs in order to improve the quality of resident care. Performs work in accordance with the DMVA general safety rules. Immediately reports all workplace injuries or accidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to the supervisor. Attends all assigned QAPI training. Follows established policies and procedures in support of QAPI efforts. Supports QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings. Follows standard precautions. Uses required personal protective equipment such as gloves, masks, gowns, goggles, face shield, hearing protection, foot protection, etc. Performs other related work duties as required. |
