Position Purpose:

This position oversees and is accountable for pharmaceutical services at a Department of Military and Veterans Affairs Veterans Extended Care Facility.

Description of Duties:

2.5% Follow universal precautions and established safety practices. Use required personal protective equipment.


Support the facility QAPI plan by actively participating in QAPI efforts; identifying opportunities for improvement; leading PIP teams; managing processes; collect and analyze data; and coaching and mentoring staff in the use of QAPI tools and techniques.

Manage department staff and operations in compliance with federal and state laws and regulations, as well as recognized and accepted safe methods of administration and handling of medications and biologicals.5%

Prepare department policies and procedures for review and approval by the Clinical Services Manager, including but not limited to: procurement, storage, labeling, dispensing, administering, controlling, and disposing of drugs and medications; monitoring of medication errors, adverse reactions, automatic stop orders, drug recalls, emergency medication kits, etc. 5%

Implement, review, and update approved policies and procedures as needed. Establish a quality improvement program for pharmaceutical services.5%

Responsible and accountable for the procurement, storage, maintenance, usage and control of all drugs and biologicals. Plan and direct monthly spot checks of inventory as well as required periodic inventories in accordance with federal and state laws and regulating agencies’ requirements.5%

Provide in-service or communication to appropriate staff on formulary contents and changes.2.5%

Supervise and participate in the preparation, compounding and dispensing of all medications.5%

Oversee and participate in the entry of medication orders onto computerized medication profiles; update medication profiles as required, including checking for drug interactions, incompatibilities, and optimum drug therapy. Bill 3rd party medications as medications are dispensed. 5%

Oversee and participate in the review of individual medication profiles and histories for potential adverse reactions or interactions. Report irregularities to the Clinical Services Manager and/or Medical Director. Ensure documentation of findings on the resident’s electronic medical record.10%

Ensure that emergency boxes are checked monthly and after each use in each of the nursing units to assure proper storage and control of drugs. Also, stock the emergency after-hours drug access area as warranted per the facility policy.5%

Prepare a quarterly report to the Quality Assurance Committee on the status of the facility's pharmaceutical services including statistics, quality review, etc. Through the committee, provide oversight, consultation and instruction regarding the proper utilization of medications at the facility. May also be a member of the Infection Control Committee, Quality Improvement Committee, or other designated committees.5%

Actively participate as a member of the interdisciplinary team. Provide consultation on the use of medications and drug interactions.5%

Provide information on resident drug regimen to the receiving facility pharmacist when a resident is transferred, and with the approval of the resident or the resident's community pharmacy when a resident is discharged from this facility.5%

Conduct periodic departmental meetings as needed to review operations, disseminate information, address concerns, etc.5%

Facilitate the receipt and dissemination of drug information and public education programs from federal, state or other agencies. Conduct in-service educational programs for professional and direct care staff and pharmacy staff.5%

Consult with professional pharmaceutical associations and work with local medical consultants or staff on special cases. Represent the department on matters involving pharmaceutical services.5%

Effectively conduct performance management of staff to include the establishment and measurement of expectations/standards, staff development, mentoring, recognition, informed recruitment/selection, effective use of staff resources, attendance management, and constructive action to address inadequate performance.5%

Perform supervisory duties such as prepare and sign employee performance reviews, approve/disapprove leave requests, make recommendations for disciplinary actions, appointments and other personnel actions, etc. Interview, along with department heads, prospective employees and assist in the selection of applicants for employment.5%

Identify actual and potential safety and health hazards for residents and staff and take appropriate corrective actions.5%
