Position Purpose:

The purpose of this position is to manage all aspects of the Department of Health’s (DOH) Well Integrated Screening and Evaluation for WOMen Across the Nation (WISEWOMAN) Program in Pennsylvania (Pa-Wise) with a focus on data and evaluation.

Please note: This position is currently located in the Cardiovascular Health Section within the Division of Violence Prevention. The position will be moving to the Division of Cancer Prevention and Control to work with the Breast and Cervical Cancer Prevention Program due to both programs serving the same population.

Overview:
The PA-WISE program is a cardiovascular screening and lifestyle program that provides access to healthy living programs targeting low income, underinsured and uninsured women ages 40-64. Eligible women can participate in cardiovascular screening, education, and access to programs such as WW (Formerly Weight Watchers®), YMCA, health coaching services and other programs that would otherwise be unaffordable. The PA-WISE program also works with clinics to help them improve the clinical processes and communications between the health care team members that will result in improved quality of services delivered to clients.

This position will focus on program data and evaluation, overseeing the management of the web-based data collection system, creating and standardizing data analysis and reporting to meet CDC and program deliverables; overseeing the evaluation contractor and activities; combining data analysis and evaluation outcomes; and assisting with other program initiatives and activities. This position will also assist in the development and delivery of quality healthy behavior programs, vendor contracting, invoicing, and improving the quality of services delivered.

Data Management, Analysis and Assessment Skills:
•Collects, analyzes, and interprets data from reliable and available sources for state and federal reporting and public health program planning and monitoring.
•Demonstrates an understanding of and ability to interpret and communicate relevant and available baseline statistics, such as numbers and rates of deaths, incidence or prevalence; populations disproportionately affected; trends; and associated behavioral risks.
•Monitors trends and patterns to identify high risk target groups and priority interventions for program and policy development.
•Creates or provides data collection methods for analysis and program reporting.
•Coordinates the development and implementation of data collection, surveillance and intervention activities, and ongoing evaluation and revision of program plans.
•Coordinates the collection and cleaning of clinical and lifestyle program data into the web-based data collection system for accurate submission and reporting to meet or exceed CDC requirements.
•Oversees the web-based data collection system including testing new enhancements, working with contractors, developing validations, and troubleshooting day to day issues for users.
•Utilizes the web-based data collection system functions to manage users; assign user roles; conduct quality assurance; clean data; create queries and reports; and other functions to ensure data is collected according to protocols.
•Updates and implements data management protocols to assure that data collection, analysis and evaluation follow guidelines, use accurate and valid methodology, and preserves the integrity of the data of electronic records.
•Monitors data and conducts quality assurance checks to ensure program services are delivered following program protocols.
•Utilizes the web-based data collection system’s data checking and runs the Minimum Data Elements (MDE) files through the CDC Data Management System (DMS) at least once a month to identify and fix any data issues prior to mandatory CDC MDE submission deadlines.
•Use Structured Query Language (SQL), Microsoft Access, Microsoft Excel and/or other software for developing queries and formulas that can be run to ensure consistent data collection and valid data analysis.
•Analyze quantitative and qualitative data collected and discuss findings to determine if any programmatic changes are necessary.
•Provide recommendations regarding improvements and changes to increase effectiveness and efficiency of program activities.
•Perform high quality data analysis work for inclusion in reports, case studies, and publications.
•Investigate and present methodology to use data (including program collected data, Census, BRFSS, and other approved data sets) to monitor program initiatives and CDC WISEWOMAN and PA-WISE program objectives.

Evaluation:
•Develops methods to report and monitor program to determine if partners are meeting requirements as well as identify gaps in program services based on fiscal analysis and reporting.
•Coordinates evaluation of public health programs, evidence-based interventions, and the improvement of preventing cardiovascular disease (CVD), improving CVD preventive care through enhanced data collection, and strengthening linkages between clinical and community resources.
•Assists in coordinating all planning, implementation, and evaluation of goals and objectives established by the grant.
•Assist in the development and implementation of evaluation activities including drafting evaluation purpose, creating data collection tools, analyzing outcomes, making observations from outcomes, and preparing recommendations.

Program, Contract, and Budget Management:
•Prepares and develops any federal grant progress or annual reports within established time frames.
•Develops and maintains expertise related to program.
•Implements and accomplishes goals and requirements established by the DOH’s and CDC’s program guidelines.
•Updates program, policy and procedure manuals as needed or requested to ensure continuity of program services.
•Develops contractual documents (work statements, scoring criteria, evaluation methodology) to create funding announcements to engage partners for public health programming.
•Makes recommendations on changes in program plans to ensure they meet the needs of target populations by using current data and ensuring changes are in accordance with existing federal, state, and departmental policies.
•Participates in monthly CDC project officer calls, other applicable conference calls and meetings, success story development with CDC staff, and other CDC evaluation or program data collection as needed.
•Monitors program adherence to performance goals, identifies potential issues or barriers, develops recommendations, and implements solutions.
•Serves as a DOH representative to various state and federal coordinating groups, advisory boards, and committees, as requested.
•Coordinates preparation, development and negotiation of grant agreements and contracts, work statements, budgets, progress reports, work plans, publications and other documents using DOH procedures and meeting CDC requirements.
•Reviews contracts and invoices for compliance and establishes compliance mechanisms, including making site visits when necessary.
•Secures funding for the DOH by writing, developing and submitting applications for federal, state, and local resources to improve the health of all Pennsylvanians.
•Provides oversight of the evaluation contractor progress including managing timeliness, deliverables, and reporting of evaluation outcomes for program improvement efforts.
•Participates in preparing and evaluating proposals (RFA, RFP, Master Agreements, ITQ, or other contractual documents) to perform program services that ensure all services can be effectively and efficiently delivered according to program goals.
•Displays proficiency in the use of Microsoft software including Visio, Word, Excel, OneNote, PowerPoint, Publisher, Outlook, and other software to create reports, analyze data and present outcomes. Display capacity to learn the web-based medical records system.

Communication:
•Prepares and delivers formal public presentations and trainings upon request.
•Develops and/or reviews educational materials for relevance, literacy level and cultural sensitivity.
•Attends national and state meetings as requested.
•Prepares and disseminates materials such as fact sheets and articles for public health newsletters and other publications to educate the public.
•Collaborates with national, state, and local organizations in the public, private, professional, and voluntary sectors to develop policies and programs.
•Prepares success stories individually or in conjunction with other staff or contractors, and/or in collaboration with CDC to promote successes in the Program.
•Demonstrates professionalism in conversations and actions with other staff, supervisors, contractors, the public, and funders. Treats all in a fair and equitable manner.
•Creates and maintains partnerships to coordinate with other chronic disease prevention and health promotion programs to further program goals. This includes identifying and collaborating with delivery system partners; and partnership building with community organizations, committees, task forces, and state and federal organizations.
•Provides technical assistance to professionals throughout the Commonwealth and the DOH using program expertise related to heart disease and stroke.
•Coordinates with other PA-WISE program staff to ensure that program operations succeed, data submission deadlines are met, and work plan goals and objectives are being updated and appropriately managed.


Quality Improvement:
•Conducts site visits and formal audits to monitor program delivery and grantee and partner agreements. Solicits feedback from partners on ways to improve program delivery.
•Maintains compliance with federal and state policies and DOH procedures; monitors grantee performance related to resource management, partnership development, and program delivery.
•Monitors contractor and grantee work to ensure activities are completed and invoicing accurately reflects work completed to ensure timely liquidation of contractors’ budgets.
•Develops and implements strategies for continuous quality improvement and develops and implements community health improvement and strategic plans.
•Develops outcome measures for compliance with federal grant funding.
•Maintain compliance, report compliance issues, develop and implement recommendations, and participate in annual reviews regarding Health Information Portability and Accountability Act (HIPAA) and Health Information Technology for Economic and Clinical Health (HITECH) regulations with all contractors accessing protected health information (PHI). Participate in any HIPAA training required by the Department.

Other:

•Travels throughout the Commonwealth and outside the Commonwealth as needed and upon request.
•Public Health Emergency Response – This position may be given an assignment to support the public health emergency response mission of the Department of Health. Emergency assignments could at times be received with little or no advance notice and may continue for any period of time – ranging from a few hours to several weeks, depending upon the nature of the circumstance. The emergency assignments may at times be performed under adverse conditions and may require travel to another site other than the employee’s assigned headquarters for periods of time.
•Other related duties as assigned.

This position reports to the Cardiovascular Health Section Chief. Work is assigned both orally and in written form. Assignments are performed independently based on knowledge of standards and procedures. Supervisor reviews on-going work progress after completion or if there is a problem or exception. Work is reviewed for consistency with mission, goals, and performance objectives. Regularly scheduled meetings are held with employee to review progress toward meeting work objectives, and to convey updated information. Supervisor schedules additional meetings as necessary to discuss and review progress on specific work projects.

This position independently assumes responsibility for program development, implementation and evaluation of assigned strategies and initiatives. Decision making occurs following interpretation and analysis of policies and procedures. Some decision making involves addressing new and unique issues and problems that may not have been addressed in the past. The employee is responsible to find information to develop solutions or approaches. Position provides technical or administrative services and coordinates activities with others outside its work unit.

This position does not supervise, manage, or serve as a lead worker to other employees.
