Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. This position will work with State Correctional Institutions (SCI), Bureau of Community Corrections (BCC), and Parole staff to coordinate services (e.g., psychosocial counseling, medication-assisted treatment, primary care appointments, etc.) for individuals suffering from serious mental illness, medical conditions and/or substance use disorder, particularly opioid use disorder. |

Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. (35%) - Thoroughly researches and analyzes records/files on all new receptions. Based upon this data, and in conjunction with the Corrections Evaluation Supervisor and Corrections Counselor, develops an Individualized Treatment Plan with the reentrant designed to address and correct problem areas significantly related to the reentrant’s psychiatric needs and the reasons for his/her referral to community corrections. Provides feedback to the Corrections Evaluation Supervisor and Community Corrections Center Director on assigned cases. Obtains information on programs available at other SCIs across the state including Mental Health (MH) and Special Needs Units to be utilized in discharge planning. In conjunction with supervisory staff, reviews policy and programs annually. (30%) - Maintains a presence at Alcohol and Other Drug (AOD) facilities and BCC MH facilities and serves as an active member of the treatment team; providing counseling, case management, and assisting in the formulation of treatment plans for reentrants assigned to centers. Assesses reentrants’ psychosocial strengths, weaknesses, and social work service needs and eligibility for services. Ensures needed benefits and services are obtained in accordance with assigned reentrant’s treatment plans for those assigned to AOD and MH centers. Assists the Corrections Evaluation Supervisor to provide necessary information regarding a reentrant’s mental health and AOD status and obtains an appointment for mental health evaluations for reentrants released at their minimum up to their maximum. Initiates, researches, and follows up with Community Corrections referrals. (25%) - Complies with the Continuity Care Policy and establishes contact with Single County Authorities/Community Mental Health Centers. Performs related work that may be required beyond the normal specifications listed above, as determined by the needs of the BCC, such as serving as the coordinator of a specified program or temporarily assuming duties of a caseload providing individual and group counseling services. This will include, but not be limited to, making necessary referrals the Bureau of Community Corrections Psychiatrist and Psychologist. Assumes responsibility for the parole planning process, including re-entry groups, and coordinating planning between multiple departments, such as Parole, Dept. of Corrections (DOC), and the Office of Vocational Rehabilitation (OVR). Conducts other treatment and re-entry related assessments as needed. (5%) - Participates in mandatory DOC and other training designated to improve counseling skills and techniques and the ability to perform one’s overall duties more effectively. Participates in the orientation of new staff members and occasionally may be required to conduct a community speaking engagement. Prepares and conducts training for other staff members about medication assisted treatment and mental health services. (5%)- Performs other related duties. ESSENTIAL JOB FUNCTIONS 1.
