Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

Provide non-professional nursing care to the residents and their quarters in a Department of Military and Veterans Affairs veteran’s home ensuring adherence to HIPAA standards.

Description of Duties:

Under the direction of licensed professional staff the Nurse Aide performs the following duties:

Contribute to residents’ Plans of Care by attending and actively participating in Interdisciplinary Team meetings to discuss the treatment plans and development of individualized interventions and methods to approach resident specific goals for their rehabilitation or maintenance. Observe residents’ conditions and report changes to professional nursing staff. Answer call bells and assist resident with care needs. Make rounds throughout tour of duty and participate in shift-to-shift report with personnel.

Perform or assist residents, in accordance with their Plan of Care, to perform Activities of Daily Living such as bathing, toileting, transfers, including in/out of bed, on/off toilet, ambulation, active and passive range of motion, shampooing, grooming, oral care, shaving, fingernail care such as cleaning, clipping, and filing. Participate in and completes Restorative Nursing Programs (RNP) as per resident’s Plan of Care. Give back
rubs and re-positions residents. Distribute and collect meal trays, feed residents, and assist with meals and nourishments as necessary. Distribute water and between-meal nourishments and assist with retraining feeding patterns.

Apply safety devices, anti-embolism stockings and boots, and elastic bandages. Apply over-the counter: lotions, creams, and ointments used for preventative skin care. Apply warm and cool devices, such as cool cloths or k-pads as directed. Gather and report pertinent data such as vital signs, height, and weight. Assist residents with opening, reading, and disposing of mail as applicable.

Transport and position residents by wheelchair, geri chair, stretcher, etc. in accordance with Department policy. Escort residents to outside appointments as necessary. Monitor the residents as needed outdoors for safety.

Record all care and services performed for the residents on appropriate forms ensuring documentation complies with all regulatory and State Veterans Home (SVH) requirements.

Make and change linens for occupied and unoccupied beds. Distribute clean linens and put away in proper areas of resident bedrooms, including their clean personal clothes.

Collect specimens as needed such as urine, feces, or sputum. Collect and empty personal laundry, dirty linen, and waste containers to proper areas and dispose of trash into trash dumpsters. Collect and dispose of infectious waste into proper storage areas. Empty urinals and bedpans. Keep utility rooms clean, neat, and organized.

Restock and label resident supplies such as toiletries, urinals, toothbrushes, basins, etc. Pick up and/or deliver supplies for the nursing unit.

Assist residents with room tidiness, cleanliness, and organization by maintaining an orderly/neat homelike environment for the residents.

Maintain a safe environment in the resident’s room by removing and addressing with the Nursing Supervisor on duty, hazards such as electrical extension cords, personal effects such as glass items or throw rugs, clutter, and other obstacles. Report unusual behavior, safety concerns, defective equipment, and unsafe conditions immediately to the Nursing Supervisor on duty. Monitor the front door/other exit doors as needed for resident safety.

Pack/unpack resident belongings upon admission/discharge/room change. Review empty beds for room readiness prior to new admissions/room transfers. Introduce residents to roommates, other residents, and to staff. Record all personal belongings in accordance with SVH policy brought in or acquired later; prepare clothes for marking by Housekeeping Services; and familiarize residents with the physical layout of the facility.

Maintain positive and constructive working relationships with co-workers and remain aware of the needs and sensitivities of others. Conduct job duties in compliance with established BVH and SVH policies, processes, and procedures.

Maintain and improve job skills and competencies by participating in and attending staff training programs in order to improve the quality of resident care.

Perform work in accordance with the DMVA general safety rules and safety rules for this position. Immediately report all workplace injuries or incidents to the supervisor; and/or immediately correct any safety hazards in the work area or reports same to the supervisor.

Support the facility QAPI efforts by attending all assigned QAPI training; following established policies and procedures in support of QAPI efforts; participate in PIP teams as assigned; sharing ideas for improvement in the organization; and supporting QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

Perform other related duties as required.

Follow universal precautions. Use required personal protective equipment such as gloves, masks, gowns, goggles, face shield, etc.

Essential Functions
