Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

The employee in this position is responsible for scheduling all outside appointments for both Personal Care and Nursing Home residents of the Southeastern Veterans Center.

Description of Duties:
Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor.

Schedule all medical appointments for both Personal Care and Nursing Home residents, e.g., medical, dental, podiatry, x-ray, optometry. Maintains a tickler file for all such visits. Work with hospitals and outside providers in coordinating short procedures, surgeries, tests, and complex specialty studies. Obtain consent forms from provider. Work with staff nurses to obtain signatures and approvals for surgery. Complete this function within a one week turnaround.

Complete appropriate paperwork from physician (Pre-Operative form); fax to provider doing surgery. Complete this function within one week turnaround. Keep a log of all interactions related to all surgical procedures.

Coordinate with the Nursing Supervisor and/or Nursing Management to schedule date, time, and location for procedures, surgeries. Arrange transportation and staff coverage with medical escort aides and other nursing staff.

Coordinate consultations with outside specialty physicians as ordered by the facility attending physician. Obtain release of information form for resident signature or power of attorney to obtain copies of any labs or studies pertinent to the specialty consultation. Follow-up to have information picked up by transportation or escort aides.

Prepare Patient Registration Health History forms for all new residents prior to their first appointment with an outside provider.

Maintain current and up-to-date appointment books.

Log all appointments for all residents in the Nursing Home and all residents in the DOM/Personal Care area. Maintain a log of outside services, which includes x-rays, ambulance transport, dental, optometry, and podiatry for DOM/Personal Care.

Function as liaison between SEVC and all community hospitals, federal VA hospitals, physician offices, and contract providers.

Compile and complete trip sheet every Thursday for the following week. Notify all staff of any preps, consents, test results, or special instructions.

Complete list for bagged meals needed by those residents who are absent from facility during meal times due to scheduled medical appointments. Keep the Dietary Department informed.

Retrieve and fax material as needed. Responsible for distribution of all labs or test results to appropriate resident areas.

Review bills from all doctor and dental visits, non-emergency ambulance transport company, and mobile x-ray company as requested.

Plan, coordinate and, communicate with the nursing and transportation departments concerning medical, off-grounds appointments. For maximum scheduling efficiency, maintain effective daily communication with the transportation department. Plan and coordinate appointments in a similar manner with other VA hospitals and agencies including the on-grounds Coatesville VA Mental Health Clinic.

Coordinate care through the federal VA System. Work with Revenue and Social Services from CVA to assure residents are entered into the federal VA system.

Notify staff nurse to prepare 513 form and doctor. Fax to the Patient Transfer offices at the federal VA in Philadelphia, Wilmington, and Coatesville. Works with the federal VA facilities to coordinate and schedule appointments at the appropriate federal VA facility.

Responsible for coordination of resident insurance information. Notify Social Services of the need for a local primary doctor as required.

Schedule first initial visit with the primary physician.

Schedule appointments with the specialty MD or tests as ordered by the primary care physician.

Responsible for calling primary doctor for referrals and communicate what procedure is requested, reason for the request, and where test is to be scheduled. For pick-up of referrals as required, make transportation arrangements.

Obtain additional referral for hospital for surgery. Responsible to notify the HMO insurance company and obtain the referral which includes the procedure code and the pre-cert numbers for pre certification. Keep a log of all referrals and numbers of visits for each referral and expiration date.

Coordinate Medicare and other insurance information with Social Service, Medical Records and the Revenue Department.

Coordinate with nursing staff regarding the necessity of a transportation aide. Complete Medical Appointment Schedule reflecting assignment of the medical escort aides. Notify transportation staff of any changes in assignments or trips, hospital discharges, or emergency appointment needs. Revise daily transportation needs as required.

Notify Social Service of any expired insurances as needed.

Coordinate transportation with ambulance service for non-emergency appointments if SEVC transportation is not available. Coordinate need for wheelchair van, regular ambulance, and special needs of residents.

Notify staff nurse to advise the medical doctor of the need to complete the Physician Certificate of Medical Necessity.

Schedule consultations with contract psychiatrist and psychologist as needed.

Relieve switchhboard operator as necessary.

Support the facility QAPI efforts by attending all assigned QAPI training; following established policies and procedures in support of QAPI efforts; participate in PIP teams as assigned; sharing ideas for improvement in the organization; and supporting QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.

Performs work in accordance with the DMVA general safety rules and safety rules for this position. Immediately reports all workplace injuries or incidents to the supervisor; and/or immediately corrects any safety hazards in the work area or reports same to supervisor.

Perform other related duties as needed.

Create and determine schedules for all medical appointments, coordinating with transportation. Determine number of appointment schedule will allow. Obtain guidance from supervisor when needed.
