Position Purpose:

The purpose of this position is to manage all aspects of the Department of Health’s (DOH) Heart Disease and Stroke Program funded by the Centers for Disease Control and Prevention (CDC).

Description of Duties:

Overview:
The goal of the Heart Disease Program is to promote blood pressure control and access to care among those with hypertension. The goal of the Acute Stroke Program is to lead quality improvement efforts to improve the care continuum for stroke patients in Pennsylvania. Efforts include establishing a learning collaborative with designated stroke centers and local EMS in a targeted region of the state and leading process improvement and quality improvement efforts to improve stroke care in PA.

This position will focus on the execution of grants, oversight of activities, monitoring of progress, preparation of reports, and ongoing communication with partners. The program administrator will provide grant management to community-based organizations to assist with program goals.

Contract and Budget Management:
• Coordinates development, negotiation, and preparation of grant agreements, contracts, work statements, budgets, reports, and publications following DOH and CDC requirements.
• Reviews contracts and invoices for compliance and establishes monitoring mechanisms, including site visits as needed.
• Develops program budgets that follow established federal funding guidelines.
• Secures funding by preparing and submitting applications for federal, state, and local resources.

Program and Data Management:
• Prepares and submits federal grant progress and annual reports on schedule.
• Maintains program expertise and implements DOH and CDC program goals and requirements.
• Manages all program and evaluation activities to meet CDC expectations.
• Updates program policies and procedures to maintain service continuity.
• Develops contractual documents (work statements, scoring criteria, evaluation methods) for funding announcements.
• Builds and maintains partnerships with health systems, community organizations, and government agencies to advance program goals.
• Recommends program changes based on current data and alignment with federal, state, and departmental policies.
• Participates in CDC calls, meetings, and required evaluation or data collection activities.
• Monitors performance, identifies barriers, and implements solutions.
• Develops program training and educational materials.
• Provides technical assistance across the Commonwealth and DOH.
• Travels as needed within and outside the Commonwealth.
• Represents DOH on state and federal groups and committees.
• Guides development of data collection, surveillance, and intervention activities, including ongoing program evaluation.
• Collects, analyzes, and interprets data for state and federal reporting and program planning.

Communication:
• Delivers public presentations and trainings as requested.
• Develops and reviews educational materials for relevance, literacy, and cultural sensitivity.
• Attends national and state meetings as assigned.
• Prepares and disseminates public health materials such as fact sheets and newsletter articles.
• Collaborates with partners across sectors to develop policies and programs.
• Prepares success stories independently or with staff, contractors, or CDC.
• Demonstrates professionalism and equitable treatment in all interactions.
• Builds and maintains partnerships to support chronic disease prevention initiatives.
• Provides technical assistance using program expertise.
• Represents DOH on coordinating groups, advisory boards, and committees.

Quality Improvement:
• Conducts site visits and audits to assess program delivery and partner compliance; gathers feedback for improvement.
• Ensures adherence to federal and state policies and monitors grantee performance.
• Reviews contractor and grantee work to verify accurate invoicing and timely use of budgets.
• Develops and implements continuous quality improvement strategies and community health improvement plans.
• Defines outcome measures to meet federal grant requirements.

Evaluation:
• Monitors contractor performance, invoices, and achievement of goals, reporting issues to the Program Manager.
• Develops reporting methods and monitors program activities to assess compliance and identify service gaps.
• Coordinates evaluation of public health programs and evidence-based interventions to prevent cardiovascular disease, improve preventive care, and strengthen clinical community linkages.
• Oversees planning, implementation, and evaluation of grant established goals and objectives.

Analytical/Assessment Skills:
• Collects, analyzes, and interprets data for reporting and program planning.
• Recommends program changes based on data and policy alignment.
• Understands and communicates relevant baseline statistics, trends, populations at risk, and associated behavioral factors.
• Identifies high risk groups and priority interventions through trend monitoring.

Other:
• Travels within and outside the Commonwealth as needed.
• Public Health Emergency Response – This position may be given an assignment to support the public health emergency response mission of the Department of Health. Emergency assignments could at times be received with little or no advance notice and may continue for any period of time – ranging from a few hours to several weeks, depending upon the nature of the circumstance. The emergency assignments may at times be performed under adverse conditions and may require travel to another site other than the employee’s assigned headquarters for periods of time.

Decision Making:

This position reports to the Cardiovascular Health Section Chief. Work is assigned both orally and in written form. Assignments are performed independently based on knowledge of standards and procedures. Supervisor reviews on-going work progress after completion or if there is a problem or exception. Work is reviewed for consistency with mission, goals, and performance objectives. Regularly scheduled meetings are held with employee to review progress toward meeting work objectives, and to convey updated information. Supervisor schedules additional meetings as necessary to discuss and review progress on specific work projects.

This position independently assumes responsibility for program development, implementation and evaluation of assigned strategies and initiatives. Decision making occurs following interpretation and analysis of policies and procedures. Some decision making involves addressing new and unique issues and problems that may not have been addressed in the past. The employee is responsible to find information to develop solutions or approaches. Position provides technical or administrative services and coordinates activities with others outside its work unit.

This position does not supervise, manage, or serve as a lead worker to other employees.
