Position Purpose: Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. The purpose of this position is to coordinate mental health services between psychiatry, psychology and nursing. This is accomplished through participation in a variety of mental health programs, to include medicating psychiatric inmates, monitoring medication compliance and providing education, triaging patient mental health needs, and promoting and supporting a therapeutic environment. |
Description of Duties: Describe in detail the duties and responsibilities assigned to this position. Descriptions should include the major end result of the task. Example: Types correspondence, reports, and other various documents from handwritten drafts for review and signature of the supervisor. Monitor medication compliance by running and reviewing pharmacy reports of medication compliance. Notify psychology and psychiatric staff of inmate medication non-compliance in order to coordinate counseling session. Conduct med compliance groups in RTU. Discusses medication compliance individually with inmates. Verify Psychiatric Observation Cell (POC) orders, and ensure orders are followed regarding meals and clothing. Obtain a shift to shift report from staff on previous shift on POC admissions. Determines when inmate was last seen and medications being taken. Reports to the medical provider. Verify POC documentation for policy compliance and ACA standards. Complete daily rounds in POC with providers. Communicate with Program Review Committee (PRC) for all POC admissions and discharges. Obtain clearance of PRC staff for inmate to return to the block. Track statistics for Monthly POC admission report. Maintain tracking system for EKGs and laboratory tests that are due. Obtain labs and have the medical provider review results. Prepare medical charts for Psychiatric Review Team (PRT). Schedule inmates for psychiatric and psychology providers prior to the PRT meeting. Attend PRT (RHU, SRU, SMU) meetings. Conduct psychiatric sick call line and triages psychiatric sick call slips for appropriate referrals. Monitor new intakes with psychiatric referrals to ensure all inmates have access. Verify accuracy of mental health classification, i.e. A, B, C, D upon reception and compares to the medical record documentation in the chart. Notifies appropriate staff of any discrepancies. Schedule inmates for PRT with stability code changes, and notifies provider of meetings. Promote and support a therapeutic environment. Oversee and provide direct nursing care and consultation to patients as required. Lead, guide and train employees performing related duties. Complete and implement corrective action plans as appropriate. Is proactive in performance improvement activities as demonstrated by participating in the QI program. Complete chart reviews as follows: oAnnual mental health informed consent document on file for psychiatry? oITP specifies frequency of psychology and psychiatry contact (not just PRN)? oICD code(s) and GAF score recorded? oIf inmate’s diagnosis meets definition of SMI, Stability D code is documented? oDocumentation that psychiatrist/psychiatric CRNP attend PRT meetings? oPsychiatrist/CRNP completes parole evaluations for MH/ID roster inmates? oDC-560 note(s) show that Psychiatry in conjunction with Psychology offer out of cell contact at least once every 90 days for C Stability inmate and once every month for D stability? oSuicide risk indicators checklist signed by nursing/psychology staff and includes a plan? oEvidence of psychiatric contact in refusal to eat/drink cases? oAIMS assessment is completed? oDoes the chart need blank progress sheets? oAre there any orders to review, or to be signed? Assist with re-entry coordination. Participate on the Suicide prevention committee. Schedule parole evaluations in conjunction with scheduler. Ensures inmates comply and evaluations are completed and distributed. Schedule psychological emergencies with provider and places inmate on schedule. Maintain logs of inmates returning with in one to two weeks for both psychiatry and psychology. Inmates are to be accounted for and seen within the two week period. Investigate and answer inmate requests regarding concerns and whether they need to be seen. Ensure completion of a DC-564 for GBMI Inmates for admission to a state hospital, if required. Ensure the form is completed, routed, and sent. Communicate with security staff in RHU for conducting psychiatric visits by providers. Identify and maintain professional boundaries to preserve the integrity of the therapeutic process and relationship. ESSENTIAL FUNCTIONS 1. Maintain a valid and active license to practice as a registered nurse issued by the PA State Board of Nursing. 9. Be mobile over all types of physical surfaces and levels 10. Visually observe inmates’ activities 11. Hear and identify sounds (voices, whistle, buzzer, lock release, scuffling, sudden quiet) 12. Attends and participate in all mandatory training and/or testing. 13. Work overtime in compliance with provisions of the Collective Bargaining Agreement. 14.Travel as required |

Work assignments are made both verbally and in writing. Shift work is designated to each shift regardless of the staff assigned. Individual assignments are assigned to each staff member and are due at varying times. The Nursing Supervisor or Correctional Health Care Administrator directly supervises the performance of all R.N.s. Skills and performance are judged by comparison to and evaluation of the achievement of preset objectives. These goals and objectives are reviewed at least annually with supervisory staff and a rating given. |
