Position Purpose:
Describe the primary purpose of this position and how it contributes to the organization’s objectives. Example: Provides clerical and office support within the Division to ensure its operations are conducted efficiently and effectively. 

Under the general supervision of a professional supervisor, is responsible for providing a full range of services to individuals who are blind/visually impaired. Services are designed to assist individuals in their homes and communities to work and maintain independent lives.

Description of Duties:

Perform work as a Social Worker for the Office of Vocational Rehabilitation (OVR), Bureau of Blindness and Visual Services (BBVS)with individuals (children/youth/adults) who are blind, visually impaired and deaf-blind in an assigned geographic area.

Develops and maintains referral sources by personal contact to assure that potential BBVS customers are identified. Provides consultation and acts as liaison to the staff of public and private agencies by responding to inquiries, visiting community agencies and organizations, providing in-service training on OVR/BBVS and conferring with staff in order to promote close working relationships, cooperative efforts and effective use of public and private resources.

Works with children/youth /adults in achieving safety and independence within their homes and community. Services include vision enhancement, advocacy, counseling, information and referral, transition services, referral for Orientation and Mobility Services, referral for Vision Rehabilitation Therapy.

Conducts a comprehensive interview with the applicant/parent or guardian to gather relevant information regarding the nature of their disability, education, psych-social factors, financial information and other data needed to complete the assessment and initiate the rehabilitation process with OVR/BBVS.

Explains eligibility requirements, scope of services, confidentiality, financial guidelines and appeal rights. Determines eligibility for services by obtaining and analyzing essential medical, ophthalmologic and financial data. In consultation with the customer/parent or guardian determines an appropriate goal and develops and approves a mutually agreed upon Service Plan. The Service Plan outlines the goals, identifies customer and agency responsibilities, identifies similar benefits to be utilized and outlines the services to be provided. Coordinates and implements planned services.

Provides counseling and guidance, referral to other agencies/services, advocacy, information on diagnostic services available, information/referral for Vision Rehabilitation Teaching, information/referral for Orientation and Mobility Services for children/youth/adults.

Accesses and updates participant records using a web-based application. Utilizes a web-based application to access participants referred for services.

Enters detailed record information directly into Commonwealth Workforce Development System (CWDS). This includes, but is not limited to, entering case progress notes, entering cost service information into a service authorization, reviewing and customizing online letters, accessing open encumbrances online, viewing invoices and reimbursements online and canceling if necessary, determining what equipment needs to be tagged and entering this information into the online service authorization.

Assumes the secondary case managers within the CWDS with real-time access to the online cases.

Travels in and out of district to attend meetings with customers, providers, employers and community groups. Attends training sessions, participates on work groups and attends statewide conferences.

Performs other duties related as assigned.

Decision Making:

Determines eligibility, services to be provided, approval of service expenditures and closure decisions.

Supervisory review of casework is conducted on a quarterly basis to ensure adherence to agency guidelines, goals and objectives.

Expenditures for services which are classified as sole source, individual consideration or those requiring a waiver for training or financial eligibility must receive signatory review by management.
