Position Purpose:

Provides rehabilitation training to customers who are blind and visually impaired designed to promote optimal personal and social adjustment to blindness, leading to self-sufficient and independent living.

Description of Duties:

The incumbent in this position is responsible for evaluating and teaching coping skills and adaptive techniques to customers who are blind and visually impaired in the community. Instruction and consultation is designed to promote optimal personal and social adjustment to blindness, leading to self-sufficiency and independent living and contributing to the customer's rehabilitation process.

Provides sequence of individualized instruction by implementing experience based learning activities and supervised practice techniques in order to ensure mastery of skills and achievement of optimal independence.

Assesses customers referred for rehabilitation teaching by collecting and reviewing data, conferring with staff, interviewing customer, and observing current level of functioning in order to develop an individualized plan of instruction and/or make referral for appropriate services.

Formulates an individualized plan of instruction with customer participation by developing service objectives, course content, selecting equipment, and projection of time for completion of instruction in order to compensate for the effects of visual impairment on customer's ability to live independently.

Monitors customer progress by evaluating achievements, adjusting objectives where necessary and by planning further instruction in order to ensure that optimal benefit is derived from the instructional program leading to attainment of objectives and closure.

Motivates and supports customer and their families by providing related counseling and/or referral in order to create and maintain an atmosphere conducive to learning and adjustment.

Updates customers' records by recording statistical data, writing narratives including plan, progress, and closure reports in order to document provision of instruction and to provide required statistics for the agency.

Keeps informed on current teaching techniques and developments by reviewing relevant materials and participating in conferences and training in order to enhance individual and statewide teaching practice.

Supports staff of community programs and facilities by providing consultation and training in order to promote positive attitudes toward individuals who are blind and visually impaired and their ability to live safely and independently.

Maintains RT inventory by accounting for inventory of teaching equipment and supplies in order to keep a current listing of items on hand and items to be purchased.

This employee is supervised by the (VR Supervisor / Casework Manager), who assigns customers for evaluation and instruction in compensatory skills needed because of vision loss to ensure that they become able to function more independently and safely. Work is reviewed through weekly conference, statistical reports, and evaluation of case records.

1) Accesses and updates participant records using a web-based application

2) Utilizes a web-based application to access participants referred for vision rehabilitation therapy services

3) Make referrals for appropriate services through the CWDS (Commonwealth Workforce Development System)

4) Enters case progress notes into CWDS in a timely manner so they can be viewed/shared

5) Enters cost service information into a service authorization through CWDS

6) Assumes the secondary case managers within the CWDS with real-time access to the online cases

7) Generates correspondence as needed within CWDS
